Position Summary

 

Reporting to the Cardiac Cath Lab Manager, the Triage Coordinator – Structural Heart Team plays a key role in facilitating timely and coordinated access to advanced cardiac care for patients requiring structural heart assessment and intervention.

 

In collaboration with Interventional Cardiologists, Cardiac Surgeons, Nurse Practitioners, and other members of the multidisciplinary team, the Triage Coordinator is responsible for receiving and reviewing referrals, ensuring complete and accurate patient information, managing the triage and booking process, and communicating effectively with patients, families, referring physicians, and internal departments.

 

The Triage Coordinator supports the operational flow of the Structural Heart Program by maintaining accurate databases, coordinating procedural logistics, and ensuring patient readiness.

 

 

Key Responsibilities

  • Collaborate closely with Interventional Cardiologists, Cardiac Surgeons, Nurse Practitioners, anesthesia, critical care teams, inpatient units, and referring centers.

  • Receive and review referrals from within THP, from regional hospitals and outpatient offices. 

  • Coordinate pre-procedure diagnostic testing and clinic consultations 

  • Monitor the structural heart waitlist, updating the clinical team on changes in patient condition and adjusting bookings as needed.

  • Coordinate and communicate procedural details with patients, families, referring physicians, and hospital units.

  • Communicate cardiac medication instructions pre-operatively in collaboration with the NP/physician team.

  • Coordinate with industry representatives to provide clinical support as required.

  • Support data collection, entry, and reporting for CorHealth Ontario databases and internal systems.

  • Participate in data analysis, outcomes tracking, and preparation of quality and performance reports.

  • Contribute to quality assurance and continuous improvement initiatives within the Structural Heart Program and across the LHIN regional network.

  • Provide triage coverage and support to other cardiac triage offices as needed.

  • Maintain effective communication and collaboration across all levels of the cardiac health program.

  • Promote the overall mission, vision, and strategic objectives of Trillium Health Partners’ Cardiac Health Program.

 

Key Qualifications

    •    Current registration with the College of Nurses of Ontario (CNO) required.

    •    Bachelor of Science in Nursing required

    •    Completion of Critical Care Certificate or CCNC required

  • 3–5 years of recent experience in cardiac catheterization lab, structural heart programs, and/or critical care required.

  • Clinical knowledge of 12 Lead ECG interpretation, stress test, echo, perfusion scans, cardiac CT, coronary angiogram, all structural heart procedures
  • Strong critical thinking, organizational, and prioritization skills.
  • Excellent communication, interpersonal, and customer service abilities.

  • Demonstrated ability to work effectively within a large interdisciplinary team, including collaboration with regional referring centers.

  • Proven ability to function independently to full RN scope of practice while maintaining accountability within team-based care models.

  • Competence in data management and familiarity with CorHealth Ontario reporting standards.

  • Demonstrated positive attitude, enthusiasm, initiative, and reliability with a strong attendance record.

  • Commitment to professionalism, patient-centered care, and continuous quality improvement.

  • Positive attendance and work record is essential
