In close cooperation with PATH Center staff, coordinates services for the NYSDOH AI-funded Retention and Adherence Program. Oversees program enrollment, outreach, marketing, and recruitment. Supervises the RAP Medical Case Manager and Outreach Specialist. Works with the Family Program Director (aka Program Director) to hire, train and retain program staff. Conducts HIV counseling and testing alongside other program staff. Provides medical case management to enrolled patients as needed. Completes program narrative reports, data review and QI projects. Represents PATH on external committees.
• Oversees the administration and execution of the RAP grant at The Brooklyn Hospital PATH Center
• Participates in all NYSDOH AI grant-related meetings and leads internal RAP meetings for the team
• Markets the program to recruit (internally and externally) RAP-eligible patients
• Supervises and guides the RAP Medical Case Manager and Outreach Specialist in providing services to RAP-eligible patients
• Ensures RAP grant deliverables are met and coordinate with the team on QI project and initiatives
• Identifies, schedules and participates in community outreach events, including ordering supplies, staffing and documenting outcomes
• Oversees scheduling and implementation of field outreach activities
• Completes monthly narrative reports and CFAs in collaboration with Program Director
• With Program Director and Executive Director, ensure staff are hired, trained, evaluated and retained
• Collaborates with Special Projects Manager on QI and outreach projects/events
• Participates in HIV-related regional meetings and events
• Supports PATH CAB meetings and events as needed in collaboration with Special Projects Manager
• Participates in grant writing that includes but is not limited to applications, reports, program promotion and development
• Participates in community outreach and networking including attendance at virtual and in person meetings and events
• Collaborates with community based organizations to identify referral resources for program patients
• Completes data entry forms and data collection for Data Analyst
• Participates in PATH Center CQI activities
• Completes monthly trainings to enhance knowledge and provision of program services
• Participates in monthly multidisciplinary case conferences
• May conduct and oversee HIV counseling and testing as needed while working closely with Manager, Special Projects who oversees HIV testing for PATH and TBHC.
• Identifies opportunities to create linkages to community-based agencies
• Conducts PrEP and PEP services as needed including PEP ED follow-up calls and enrolling PrEP-AP and Advancing Access programs
• Additional duties, as required.
