Program Manager/ Registered Nurse

The Program Manager / RN for the S-O-R Program is a licensed nurse (RN or LPN) who serves as both the lead clinician and program manager for the S-O-R Care-Management initiative under the C-C-M-P (Care Management Network) grant funded by SAMHSA.
This merged role combines nursing care responsibilities�such as triage, care coordination, and patient education�with program management functions, including supervision of staff, compliance oversight, and grant reporting. The Program Manager / RN ensures that clients, especially those experiencing homelessness and struggling with Substance Use Disorders (SUD) or other high-risk conditions, are connected to timely, high-quality medical, behavioral-health, housing, and supportive services.
Key Responsibilities
� Conduct triage assessments to determine clients� immediate medical or behavioral-health needs.
� Provide basic health screenings (e.g., vital signs, medication monitoring, wound care as appropriate within scope).
� Coordinate care plans with pain-management specialists, primary-care providers, and behavioral-health professionals.
� Offer health education, risk-reduction counseling, and follow-up support for clients with SUD or chronic health conditions.
� Document all clinical encounters and ensure adherence to nursing standards and protocols.
Program Management Responsibilities
� Oversee the daily operations of the S-O-R / C-C-M-P Care-Management program.
� Supervise care-management staff including case managers and recovery support specialists.
� Ensure compliance with SAMHSA grant requirements, agency policies, and quality standards.
� Monitor progress toward program goals and deliverables; track outcomes for high-risk and homeless populations.
� Prepare monthly, quarterly, and annual reports for funders and Hillcrest leadership.
� Coordinate partnerships with hospitals, shelters, housing agencies, and community-based organizations.
� Represent the program in meetings, coalitions, and community outreach events.
Client Support & Advocacy
� Serve as the primary clinical advocate for high-risk clients to remove barriers to care and improve outcomes.
� Facilitate referrals to medical, behavioral-health, housing, and social-service providers.
� Support continuity of care through regular case conferences and collaborative treatment planning.
Qualifications
� Current license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the District of Columbia.
� Bachelor�s degree in Nursing or related healthcare field preferred (Associate degree accepted for LPN with strong experience).
� 3�5 years of nursing or care-management experience, preferably with SUD treatment, care coordination, and high-risk/homeless populations.
� At least 2 years of supervisory or program-management experience in a healthcare, behavioral-health, or community-based setting.
� Familiarity with SAMHSA-funded programs (e.g., S-O-R) strongly preferred.
� Strong clinical assessment skills and understanding of community resources.
� Excellent organizational, communication, and problem-solving abilities.
