Purpose: The License Vocational Nurse (LVN) facilitates health care service to women and children residing in Residential Treatment Program for Women and Children in South Los Angeles. The Medical Assistant upholds the JWCH mission to improve the health status and wellbeing of under-served segments of the population of Los Angeles County. This position is funded through contracts with Los Angeles County Substance Abuse Prevention and Control (SAPC) and Sexual Reproductive Health Specialist contract (SRH).
Duties and Responsibilities:
- Performs routine care functions as prescribed by established policies and procedures within defined scope of education, training and responsibilities
- Responsible for obtaining Physical Examination and TB compliance for each patient through care coordination
- Logs medication, maintains Medical Administration Record, and audits MAR daily in collaboration with licensed nurse.
- Documents on Nextgen and PCNX electronic health record (EHR) system.
- Organizes and maintains medication cabinet and patient medication boxes; to include removing medications left by patients and packaging them for proper disposal.
- Meets regularly with patients to review medications, appointments and refills needed and refers to the nurse
. - Assists with Medi-Cal eligibility and coordinates with Enrollment Specialist for out of county transfers
- Assesses if patient has a medical provider and make change to Wesley Health Centers as appropriate. Assists in completing enrollment paperwork
- Schedules appointments in Nextgen for psychiatric, family, prenatal and postpartum health care and other medical services
- Works with patients on self-management goal under the direction of a licensed health care provider
- Performs routine clerical functions as assigned
- Takes and routes phone calls timely and efficiently
- Schedules routine and follow-up appointments
- Keeps medication room and work areas clean and stocked
- Maintains confidentiality at all times
- Helps to facilitate Sexual and Reproductive Health services per County SRH contract: Outreach and engagement to clients residing in residential program; basic overview of reproductive health; issues related to drug use and how it relates to pregnancy; general education on HIV/AIDS/sexually transmitted diseases and referrals for testing and treatment; inform clients of benefits under pregnant and parenting services (PPW); refer and link to other available services; discuss family planning and education; assists in completing Reproductive Health Form and Women’s Health History Form located within EHR.
- Education on risk of HIV, Hepatitis C and other communicable diseases.
- Assists with toxicology testing, submission and results
- Coordinates with Medical Assistant to conduct intake assessment, including Health Questionnaire with new admits and their children
- Assists with COVID-19 testing with new patient admissions and ongoing testing as needed. Keeps an inventory of rapid test
- Coordinates with Program Administrative for appointment transportation needs
- Maintains Incident Log when Paramedics are called to the facility, and follows through when patient is admitted to the Hospital and reviews discharge paperwork upon return
- Enforce all COVID-19 safety measures
- Performs other duties as assigned.
Position Qualifications
- Current and valid license for vocational nurse in State of California
- Current Basic Life Support and First Aid card from the American Heart Association,
- Able to work with challenging and resistant populations
- Ability to communicate effectively orally and in writing,
- Experience in working with patients and staff from diverse socio-economic, ethnic and cultural backgrounds,
- Cultural competent working with minority, low income and vulnerable women
- Able to work Holidays and other nontraditional workdays and hours.
- Bilingual capabilities in English and Spanish preferred
- Preferred: Registered Substance Use Disorder (SUD) Counselor: Must be certified by one of the agencies accredited by the NCCA and recognized by DHCS, which include CADTP and CCAPP; and who must adhere to all requirements of the California Code of Regulations, Title 9, Chapter 8 and Title 9.
Skills and Knowledge
- Knowledgeable of alcohol and substance addiction, systemic trauma, sexual and reproductive health, smoking cessation, medication assisted treatment and child development
- Knowledgeable of mental illness diagnosis and psychotropic medications treating mood disorders, schizophrenia and anxiety disorders
- Proficient in Microsoft Programs
- Proficient in managing Medical Administration Record
- Excellent documentation, oral and written communication skills
