Renal Nurse Navigator
Position Summary
This position will provide coverage for the Navigators for Renal patient flow and coordination as well as for the Navigators for vascular access coordination. In collaboration with the Renal Program Inter-professional team at Trillium Health Partners, the Ontario Renal Network, and community partners, this dual-role position supports patients through education, care coordination, and vascular access management. The Renal Nurse Navigator facilitates informed decision-making regarding dialysis modalities and ensures seamless transitions within the Regional Chronic Kidney Disease Program, as well as oversees dialysis access care and coordination across inpatient and outpatient settings.
Key Responsibilities
Renal Nurse Navigator- Patient Flow & Coordination
· Coordinate care for patients initiating dialysis with the inter-professional team.
· Act as the primary contact and resource for new renal patients.
· Promote independent dialysis and advocate for patient choice.
· Customize teaching approaches based on individual learning needs.
· Assist patients and families in achieving optimal self-care.
· Monitor renal inpatient volumes and acuity.
· Collaborate with inpatient and outpatient teams to ensure continuity of care.
· Facilitate communication between patients/families and the healthcare team.
· Participate in rounds, hospital committees, and nephrology planning activities.
· Support transitions through the Regional Chronic Kidney Disease Program.
· Weekend and on-call shifts may be required during high-volume periods.
Renal Nurse Navigator - Vascular Access Coordination
· Review and update patient lists for access and CRT cannulation; assist with procedures.
· Plan and coordinate blood work for IR and OR procedures.
· Process AV access referrals and support vascular surgery clinics.
· Troubleshoot complex access issues and assess AV access in clinics.
· Process IR procedure orders and coordinate appointments, provide patient education
· Coordinate PD catheter insertions at Centres of Practice.
· Perform PD flushes for diagnostic imaging recovery patients and outpatients.
· Update patient records in Epic with access-related information.
· Communicate patient status to care team members.
· Support and troubleshoot cannulation or access issues across sites.
· Coordinate and schedule cannulation training for nurses.
· Develop and deliver patient education (e.g., VA safety, Save My Veins).
Qualifications
· Bachelor’s Degree in Nursing (required or in progress).
· Current Certificate of Registration with the College of Nurses of Ontario.
· Minimum of 3 years of recent clinical experience in hemodialysis.
· CNeph(C) certification preferred.
· Experience with ultrasound-guided cannulation preferred.
· Understanding of all dialysis modalities preferred.
· Experience applying adult education principles and demonstrated teaching skills.
· Strong problem-solving and decision-making abilities.
· Excellent communication and interpersonal skills (individual and team-based).
· Demonstrated leadership and organizational skills.
· Proficiency in computer applications including MS Word, Excel, Epic, Microsoft Teams
· Membership in relevant professional organizations.
· Highly motivated and capable of working independently and collaboratively.
· Satisfactory attendance and performance record.
