What You’ll Do 

• Coordinate Transitions of Care: Contact recently discharged patients within 48 hours, arrange timely follow-up visits, document interactions, and address patient needs such as medications or resources.
• Support Care Management: Assist RN/LCSW Care Managers with patient outreach, assessments, consent, and care plan development; provide education and monitor progress toward chronic disease and pain management goals.
• Facilitate Communication and Population Health Efforts: Promote clear communication between patients and providers, support immunization and health campaigns, and connect patients to wraparound and social support services.
• Promote Education and Organizational Culture: Contribute to patient and colleague education initiatives, uphold organizational values, and participate in activities to enhance patient satisfaction.
• Ensure Quality, Safety, and Fiscal Responsibility: Support quality improvement and cost-reduction goals, follow safety and infection control protocols, and maintain patient confidentiality and compliance with HIPAA standards.

What We’re Looking For 

Required: 
• BLS Certificate from American Red Cross or American Heart Association
• Clean driving record with current NC driver’s license and approved by corporate vehicle insurance vendor for driving privileges
• Current NC LPN license or CMA certification
• High School Diploma or GED
• Associate Degree in LPN/CMA
Preferred: 
• Two (2) years of experience in health care
• Medical Terminology education
