An overview of this position entails…This position provides safe transportation of residents and equipment for the Life Adjustment Program.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Properly securing and transporting residents to various appointments such as doctor, hospital, employment, shopping, etc., ensuring residents arrive safely and in a timely manner.
2. Properly transporting equipment needed by residents, such as wheelchairs and walkers.
3. Accompany residents to appointments and act as a communication liaison between the resident, provider, and LAP health services.
4. Reminding residents of upcoming transportation times, locations, and specifications; coordinating resident load times.
5. Performing routine vehicle safety inspections and regularly clean vehicles as needed.
6. Reporting any vehicle issues or accidents immediately to the supervisor or Hilltop’s Corporate Transportation/Warehouse Supervisor.
7. Following guidelines in the Transportation Handbook located in each vehicle.

EDUCATION & EXPERIENCE
High school diploma or general education degree (GED) and one year of related work experience. Minimum 21 years of age required.
CERTIFICATES, LICENSES & REGISTRATIONS
- Valid Colorado Driver’s License (required)
- Qualified Medication Administration Personnel (QMAP) Certification (preferred)
- CPR/First Aid Certificate (preferred)
- De-Escalation Training (preferred)
- Hilltop Transportation Certification (required)
