Location: Brooklyn, NY or Hollywood, FL
Schedule: Full-Time
Salary: $250,000 - $350,000 per year DOE

Job Summary
As the Molecular Pathologist, you will lead Infinite’s molecular diagnostics and pharmacogenomics (PGx) efforts specifically tailored to residents in skilled nursing facilities (SNF), long-term care (LTC), and geriatric settings. You will be responsible for reviewing and interpreting molecular/PGx test results, integrating them into resident medication plans, working with external diagnostic labs, and helping internal clinical teams optimize treatments for an older population. Your work will support the “treat in place” model by reducing adverse drug events, minimizing hospitalizations, and improving medication‐management outcomes.
Responsibilities
- Review, interpret, and sign out molecular diagnostic and pharmacogenomics (PGx) reports, including germline and somatic testing relevant to older adults (e.g., drug metabolism genes, pharmacokinetic variants, etc.).
- Evaluate residents’ molecular/PGx profiles to guide pharmaceutical compatibility, optimize medication regimens, reduce polypharmacy, and mitigate adverse drug reactions.
- Serve as liaison to external diagnostic laboratories for referrals (sending out testing), ensuring quality, turnaround times, and appropriate assay selection.
- Oversee validation and selection of molecular assays/panels relevant to LTC populations; ensure compliance with regulatory requirements (CLIA, CAP, etc.) and lab best practices.
- Work collaboratively with facility medical staff, pharmacists, geriatricians, and nursing leadership to embed PGx insights into care plans.
- Provide training, education, and consultative support to clinicians in SNFs/LTCs on interpreting molecular data and prescribing based on PGx.
- Participate in quality assurance/performance improvement programs (QAPI), including documentation, monitoring outcomes (e.g., reduction in falls, hospitalization, ADRs), and helping facilities achieve quality metrics.
- Monitor developments in molecular diagnostics, PGx guidelines (e.g., CPIC, FDA, others), and integrate evidence-based updates into MedElite’s protocols.
- Assist with policy, documentation, and reimbursement strategy as relevant to PGx / molecular testing in LTC settings.
Requirements
- MD or MD/PhD (or equivalent) with board certification or eligibility in molecular genetic pathology, genomic pathology, or comparable subspecialty.
- Clinical experience interpreting molecular/PGx testing, ideally in older adult populations, or settings with high rates of polypharmacy.
- Familiarity with molecular technologies (NGS panels, qPCR, variant annotation, copy number, possibly structural variation) and with bioinformatics or working with variant classification sources.
- Strong clinical judgment with respect to medication safety, pharmacogenomic interaction, and resident care optimization in skilled nursing / LTC environments.
- Excellent communication and collaboration skills: ability to work with facility staff (nurses, internists, pharmacists), external labs, and administrative leadership.
- Experience in quality improvement/outcome monitoring, regulatory compliance in diagnostics.
- Ethics, attention to detail, and ability to interpret data in the context of geriatric/older adult physiology.
