The CSC Team Leader is an experienced Master’s level clinician who is trained in working with individuals experiencing first episode psychosis (FEP). He or she will be the primary contact person for clients and families and will spearhead efforts to engage clients in treatment. The Team Leader’s primary goals are to build a positive relationship with participants and assist them in developing their abilities for illness self-management. The Team Leader will work with participants using a shared decision-making process to develop and modify treatment plans. The Team Leader will provide support, education, consultation, and basic services to participants and their families. With younger individuals, work with families will be more prominent since they play a pivotal role in the individuals’ lives during adolescence and the first years of adulthood.
PROGRAM DESCRIPTION
The CSC (Coordinated Specialty Care) Early Psychosis Program is a specialized treatment approach designed to help individuals who are experiencing their first episode of psychosis. The goal of the program is to provide comprehensive, team-based care that addresses the various needs of young people (typically ages 16-30) who have recently experienced symptoms of psychosis. The program is aimed at improving long-term outcomes and minimizing the impact of psychosis on the individual's life.
JOB DUTIES & RESPONSIBILITIES:
Serve as the primary contact for clients and families, establishing rapport and trust to support engagement in treatment. Foster a positive, supportive environment to encourage participation in the CSC program.
Collaborate with clients to develop personalized treatment plans using a shared decision-making process. Regularly review and modify treatment plans in response to client progress, preferences, and evolving needs.
Guide clients in understanding their diagnosis and the process of illness self-management, empowering them with the tools and strategies needed to manage their symptoms and overall well-being.
Provide education and emotional support to families, particularly for younger clients, ensuring that family members are equipped to understand the illness and actively participate in the treatment process.
Work closely with other members of the CSC team to ensure cohesive, coordinated care. Provide guidance and mentorship to other team members as needed.
Ensure that clients are receiving the appropriate services and interventions in a timely manner. Collaborate with external providers, family members, and other stakeholders to coordinate care and support for clients.
Provide support during crises or acute episodes, ensuring the safety and well-being of the client. Respond promptly to emergencies and collaborate with the team to address urgent needs.
Continuously assess client progress and make recommendations for adjustments to treatment plans. Provide feedback to clients and families in a constructive, empathetic manner.
Performs other duties as required or assigned to assist the needs of the program or organization.
Requirements
GENERAL REQUIREMENTS:
Education/Experience
MSW, MFT, or Master’s in Counseling, or equivalent.
LCSW, LMFT, LPCC preferred.
Minimum of 4 years of experience working with individuals with psychosis, mental health disorders, or related conditions.
Experience in working with adolescents and young adults is highly desirable.
Familiarity with Coordinated Specialty Care (CSC) models or similar evidence-based treatment approaches for psychosis is a plus.
Licensure, Registration, Certification
Active BBS registration.
Valid CA driver’s license and proof of automobile insurance.
Completion of CPR/First Aid certification within the first thirty (30) days of employment.
PHYSICAL REQUIREMENTS:
Ability to drive a motor vehicle without restrictions, often in heavy traffic and frequently long distances (one hour or more) to ensure safe transport to client settings and to safely transport clients to appointments as needed.
Senses of vision, hearing and smell sufficient to regularly operate a motor vehicle, to visualize threats in the environment or risks to clients, and to operate safely in the field.
Required to occasionally lift and carry 10-20 pounds.
Regularly required to sit, stand, climb stairs (1 or more flights) and walk while in route to and during family visits.
Regularly required to sit and use computer in office settings.
