POSITION: Memory Care Coordinator 

REPORTS TO: Health and Wellness Director

POSITION SUMMARY Under limited supervision, plans, directs, and evaluates total programming and memory care  within the context and according to the guidelines set forth by the Ciel Senor Living memory  care program, Encore. Helps staff understand that emotional and social displays of care  positively affect the residents. 

ESSENTIAL JOB FUNCTIONS: 

• Supports the mission, vision, and goals of the organization, upholding and promoting  company culture and vision. 

• Directly supervises all associates that work with residents in the Memory Care program  with final authority resting with the Health and Wellness Director. 

 • Is responsible for the overall direction, coordination, and evaluation of this department. 

• Carries out supervisory responsibilities in accordance with the organization’s policies and  applicable laws.  

• Responsibilities include interviewing, hiring, and training associates in conjunction with  the Health and Wellness Director; planning, assigning, and directing work; evaluating  performance in coordination with the Health and Wellness Director; rewarding and  disciplining associates; addressing complaints and resolving problems. 

• Is completely familiar with the state regulations governing Assisted Living and memory  support care, ensuring compliance in department by regularly reviewing regulations  checklist. 

• Meets Ciel Senior Livings Expectations established for the Activities and Memory Care  departments, as applicable. • Assists in the development and/or refinement of the dementia policies and procedures. 

• Reports regulatory compliance issues to Health and Wellness Director, Executive  Director, and Regional Director of Operations. 

• Confers with the Health and Wellness Director/Executive Director regarding assessments  of residents, admissions, and participates in discussions regarding resident discharge  based on discharge criteria. 

• All charting is performed timely and in accordance with state regulations. 

• Provides timely and honest communication with families to promote good relationships  and keep them informed about resident’s condition and community events; offers  education, information, and support as needed. 

• Participates in monthly Memory Care conference call and submits required monthly  paperwork. 

• Maintains outreach and education efforts throughout the community to promote the  marketing effort.  

• Provide marketing support within the community by participating in tours, discussions  with potential residents and their families, and possible referral source visits, if deemed  necessary. 

• Provides assessments for potential residents to determine their eligibility for the Memory  Care Program, based on established admission and discharge criteria. 

• Confers with Health and Wellness Director/ Executive Director regarding assessments of residents, admissions, and participates in discussions regarding resident discharge based  on discharge criteria. 

• Obtains appropriate Memory Care admission paperwork prior to admission.  

• Conducts mini-mental or other assessments as required or has it conducted through an  outside source, in coordination with the Sales Director and the Health and Wellness  Director. 

• Performs Program Quality Assurance audits using assessment tools provided. 

• Creates Resident Histories and Profiles with family involvement, shares information with  staff. • Manages expenses within given budget parameters utilizing spend-down sheets. 

• Plans a varied monthly activity calendar within the company standards. 

• Arranges for appropriate spiritual activities as reflected by resident preferences. 

• Participates in the planning and coordination of Family Nights. 

• Posts a community calendar monthly.

 • Encourages all residents to participate in some or all of activities according to their  abilities and preferences and maintains record of resident attendance. 

• Work with the Sales Director and/or Life Enrichment Director in planning and assisting  with all community programs which involve residents, staff, and family participation. 

• Works in collaboration with the Life Enrichment Director to arrange for a variety of  entertainment to be brought to the community for the entertainment of the residents. 

• Arranges for a variety of exercise opportunities suitable for the elderly population to  enhance residents’ mobility, strength, bone mass, and social interaction.

 • Communicates with other department heads and staff to gain necessary participation in  activities as necessary. 

• Keeps sufficient activity program and craft supplies on hand for in-house entertainment. 

• Facilitates, co-facilitates, or arranges monthly Alzheimer’s support group for families and  outside community. 

• Ability to drive the community vehicle to transport Community residents, including but  not limited to providing transportation to local physician or dental offices, trips outside  for shopping, entertainment, education, and/or appointments, both non-medical and  medical, outside of the Community, as assigned. 

• Creates positive associate relations through staff appreciation programs, recognition of  excellent performance, and retention through regular feedback systems and promotional  incentive programs. 

• Observes and demonstrates appropriate caregiving, programming, and interaction skills  with the direct care staff. 

• Provides appropriate guidance for associates by planning work schedules, assigning work  duties while on shift and directing the work daily. 

• Assures responsibilities for Daily Program assignments are clearly delineated. 

• Lead in-depth Alzheimer’s/dementia care staff training for new staff; provide training as  needed and provide in-services as needed to keep staff abreast of current practices, if  deemed appropriate. 

• Arranges for required and appropriate orientation, education, training, and in-services for  staff in accordance with licensing requirements, state guidelines, and the needs of the  community.  

• Documents all training as required. 

• Maintains a high level of daily communication with staff and key personnel. 

• Stays educated on Alzheimer’s related topics (books, videos, conferences, etc). 

• Attends training courses at community and maintains up to date monthly, quarterly,  annual training through designated Learning Management System (LMS). 

• Other duties as assigned, and which relate to the success of the community and the care,  comfort, and happiness of our residents. 

Requirements

 EXPERIENCE, QUALIFICATIONS & SKILLS: 

• Bachelors degree 

• One-two years of relevant experience. 

• Ability to communicate clearly and effectively. 

• Medication Administration training/certification per state requirements.

 • Experienced Caregiver of Certified Nursing assistant certification per state requirements  

• CPR certification. 

• Must have strong understanding of the English language sufficient to read, write and  interpret administrative information. 


PHYSICAL QUALIFICATIONS: 

• Walk/Stand – must be able to continuously walk and stand. 

• Environment Condition – must be able to perform work both inside and outside. 

• Sit – sit infrequently. 

• Lift - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds. 

• Carry - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100  pounds. 

• Push - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds. 

• Climb – must be able to climb stairs on a frequent basis, with the opportunity to utilize  the elevator as an alternate resource if available. 

• Bend – must be able to bend at the waist, knees, hips, and spine on a frequent basis; may  require frequently manipulating weights of 25 pounds or more. 

• Squat – must be able to squat on a frequent basis to reach below the knee, may require  lifting of 50 pounds or less. 

• Twist or rotate – must be able to twist or rotate occasionally at the trunk, hips, knees, and  neck, manipulating weights of 25 pounds or more. 

• Reach– must be able to reach on a frequent basis. May occasionally reach overhead,  requiring manipulation of weight of 10 pounds or less, and below waist manipulating  weight of 25 pounds or more
