About the Department

The Child and Family Connections Supervisor is responsible for coordinating activities delegated by the Program Coordinator at the CFC program.  This includes but is not limited to direct supervision of a team of staff including Service Coordinators and others as needed, program planning and organization, staff training and evaluation of program services.  The CFC Supervisor is also responsible for a limited caseload and providing program education in the community.


Scheduled Hours: 37.5 hours per week

Position Duties

  • Provide Supervision to assigned CFC staff including monthly meetings, discussions, conflict resolution, trouble-shooting difficult cases and other support as needed.   
  • Lead the CQI efforts of the program by pulling monthly/quarterly reports to update CQI goals and developing additional CQI goals as requested by Program Coordinator.
  • Lead the Service Coordinator record review process monthly by conducting record reviews and supporting staff to keep correct records based on DHSs EI process.
  • Maintain an active caseload as agreed upon by the program coordinator and meet the Early Intervention system and CFC program perimeters and adhering to established timelines.
  • Collaborate with the Program Coordinator in the overall planning, organizing and evaluation of program services for children and families.
  • Provide support to the assigned team members including conflict resolution and troubleshooting difficult cases.
  • Assist in the implementation of standard operating procedures that ensure the provision of quality program services including but not limited to leading workgroups (both temporary and permanent) that improves program functions.
  • Conduct monthly meetings and performance appraisals (as required) for all supervisees as directed by LCHD and Program Coordinator.
  • Adhere to program set timelines, including but not limited to reports and schedules.
  • Effectively track your Early Intervention credentialing needs and maintain an active Service Coordination credential.
  • Promptly return all phone call messages, e-mails, faxes and other forms of communication from families, providers, program staff and others consistently within 48 business hours of their arrival.
  • Support the training of new program staff as assigned.
  • Perform other duties as assigned.

Minimum Qualifications

  • Bachelor’s degree, in a related field (health education, social work, psychology, sociology, early childhood development, etc.)
  • Case management experience preferred.
  • Requires a valid Illinois driver's license and automobile insurance.
  • Requires the ability to lift up to 30 pounds (i.e., carrying supplies, traveling with computer, etc.).
