About the Role

You will be an experienced member of the pharmacy team and you will provide comprehensive high quality pharmacy services to the areas rostered.

Patient centred care will be provided in line with local policy, best practice standards and legislative requirements resulting in safe, high quality care for the patient.

You will be asked to work in a rotational basis in different clinical areas including any ward, sub-acute care, outpatients, theatres and pre-admission clinic, mental health and cancer services. 


Classifications for this position will range from SX2 - SX5 ($94,478.80 - $106,906.80 per annum).

Your Contribution
• AHPRA registered pharmacist
• At least 3 years hospital pharmacy experience

St Vincent’s Hospital Pharmacy service provides high quality pharmacy service to St Vincent’s Hospital Melbourne, St George’s Hospital, Caritas Christi Hospice and correctional facilities managed by St Vincent’s Hospital.

Pharmacy services are decentralised and are provided from several satellite pharmacist located on wards and in treatment areas.

Position Description


St Vincent’s Hospital Pharmacy Department pharmacy services are decentralised, and are provided from several satellite pharmacies located in patient treatment areas. We provide pharmacy services to inpatients, outpatients and correctional facilities. The pharmacist is a member of the multidisciplinary 
health care team which delivers care in a patient focused manner. 


POSITION PURPOSE
This position is a member of the pharmacy team and has the responsibility of providing comprehensive high quality pharmacy services to the areas rostered. Patient centred care will be provided in line with local policy, best practice standards and legislative requirements resulting in safe, high quality care for the 
patient. The pharmacy service is decentralised and areas of service include acute and sub-acute wards, central Pharmacy, Outpatients Pharmacy, Oncology Pharmacy, theatres and pre-admission clinics, Mental health and virtual hospital services.


This position may be asked to work in different clinical areas including any ward, sub-acute care, theatres and pre-admission clinic, mental health and cancer services. If working in cancer services this role will be involved in all stages of chemotherapy manufacturing from clinical review of prescriptions, to 
preparation, checking, releasing and patient counselling. As a grade two pharmacist this position will be responsible for an additional responsibility in the 
department – on top of the direct patient load. This additional responsibility will be determined by pharmacy management and will be tailored to the skills of the pharmacist in this position. As a grade two pharmacist this position may be rostered to specialist areas (with training and competency assessment) including but not limited to partnered pharmacist medication charting, cancer services.

POSITION DUTIES
 Provides a high level clinical pharmacy services including but not limited to oncology and haematology pharmacy
 Obtains and documents full and accurate best possible medication history
 Prescription dispensing, timely supply of medications and facilitating the discharge process
 Aseptic dispensing including cytotoxic manufacturing
 Satellite pharmacy services
 Actively praricipates in the pharmacy Continuing Education program
 Actively identifies, leads and participates in continual improvement activities in the department including intervention reporting
 Identifies and advises on optimal drug therapy, drug interactions and ADRs
 Conducts thorough medication counselling
 Actively participates in eduction of other health professionals and consumer groups
 Actively participates in practice research including regularly presenting and publishing research or case studies 
 Practices in accordance with formulary restrictions and MTAC recommendations
 Participate in partnered pharmacist medication charting (PPMC) credentialing and once credentialed work on the PPMC rosters as required 
 Additional responsibilities as specified by the Senior clinical pharmacist, deputy chief pharmacist or chief pharmacist
 Actively participates in education of post and undergraduate students 
 Provides timely accurate handover to facilitate seamless transition between care areas and healthcare providers.

INCUMBENT OBLIGATIONS
 Perform duties of the position to best of their ability and to a standard acceptable to SVHM

 Complete annual clinical competencies
 Maintain skills and knowledge necessary to safely and skillfully undertake clinical work 
 Consult with peers and other experts and refer to other healthcare workers when appropriate and in a timely manner 
 Collaborate and clearly communicate with patients/clients and the healthcare team 

Participate in clinical risk management and continuous quality improvement activities as part of day-to-day work 
 Ensure consumers receive information in an appropriate and accessible format
 Actively support consumers to make informed decisions about their treatment and ongoing care
 Ensure consumers are aware of their rights responsibilities and how to provide feedback
 Protect the health and safety of self and others, complying with all health and safety related policies, procedures and directions 
 Complete required Fire and Emergency Training annually
 Complete required Workplace Culture and Equity Training annually
 Attend general hospital orientation within 3 months of commencement
 As required, comply with fit-testing and PPE requirements
 Participate in reporting and analysis of safety and quality data including risks or hazards,
 Report any hazards, near misses and incidents (regardless of whether an injury occurred or not) 
into Riskman 
 Identify and report any variance to expected standard and minimising the risk of adverse outcomes

8. SELECTION CRITERIA
8.1 ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS


 Bachelor of Pharmacy
 Registered with the Pharmacy Board of Australia


8.2 OTHER ESSENTIAL REQUIREMENTS
 At least 3 years hospital pharmacy experience
 Exhibit excellence in pharmaceutical practice skills
 Flexibility to work in all sections of the pharmacy department according to St Vincent’s Policies
 Ability to work as an experienced member of a multidisciplinary team in relation to St Vincent’s Patient Care Model
 Excellent communication skill
