Position Summary
The Per Diem Healthcare Outreach Specialist is responsible for supporting community outreach, patient navigation, and engagement activities on an as-needed basis. This role helps increase access to healthcare services by building relationships with community members, providing education about available programs, and assisting patients in navigating the healthcare system. The Specialist will represent the health center at community events, support patient enrollment efforts, and assist with follow-up activities to improve patient retention and satisfaction.
Key Responsibilities
· Conduct outreach in the community to raise awareness of health center services, including participation in health fairs, community events, and neighborhood canvassing.
· Provide linguistically appropriate education and resources to individuals, families, and community partners.
· Assist patients in scheduling appointments, completing registration, and accessing needed services.
· Support patient follow-up and engagement efforts to reduce no-shows and improve continuity of care.
· Distribute outreach materials and maintain accurate records of outreach encounters.
· Document outreach, engagement, and patient navigation activities in accordance with health center policies and reporting requirements.
· Collaborate with care teams, enrollment staff, and community partners to ensure seamless patient experience.
· Maintain confidentiality and comply with HIPAA and all applicable regulations.
· Perform other related duties as assigned.
Qualifications
· High School Diploma or equivalent required.
· Previous experience in community outreach, patient navigation, or customer service strongly preferred.
· Bilingual proficiency (e.g., Spanish, Bengali, or other language relevant to the community) highly desirable.
· Strong communication and interpersonal skills, with the ability to engage diverse populations.
· Knowledge of community resources and healthcare access issues.
· Flexible schedule, with ability to work evenings, weekends, and as-needed assignments.
· Comfortable working in community settings (outdoors, events, door-to-door, etc.).
· Basic computer skills, including use of Microsoft Office and electronic health record systems (preferred).
Working Conditions
· Per diem role, hours vary depending on organizational and community needs.
· May require travel within the community and occasional lifting of outreach materials (up to 25 lbs).
· Work is performed in both health center and community-based settings.
