What We’re Looking For
Trillium Health Resources has a career opening for a TCL Clinician to join our team! The TCL Clinician is responsible for providing care coordination/care management (treatment planning, assessment, monitoring, educating, and referral/linking) to individuals identified in the Transitions to Community Living Program (TCL) Special Needs Populations. The TCL Clinician will provide care planning with foundations in national evidence based and informed standards providing whole person care.
On a typical day, you might:
- Provide integrated care coordination/care management to members who have identified needs with behavioral health, physical health, co-occurring, co-morbid and/or multi-morbid conditions.
- Work collaboratively with the care team to ensure successful discharge/transition planning.
- Assess members’ needs who have transitioned into supportive housing to determine if additional support and services are needed for continued independent living.
- Provide education regarding available services, natural and community support, treatment options, diagnosis, internal Trillium Health Resources operations and procedures, etc.
- Review clinical documentation such as comprehensive clinical assessments and psychological evaluations to ensure appropriate recommendations for services and level of care.

Qualifications
Required:
- Master’s degree and minimum of one (1) year post-Master’s experience in a clinical setting with the MH/SU population.
- Fully licensed by the state of North Carolina as a LCSW, LPA, LCMHC, LMFT, or LCAS is required.
- Must have a valid driver’s license.
- Must reside within Trillium’s catchment area, which includes the following counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenior, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland, Anson, Guilford, Montgomery, Randolph, and Richmond.
- Must be able to travel within catchment as required.
Preferred:
- Degree in Human Services field.
- Licensure in both mental health and substance abuse.
