What You’ll Do:
The Clinical Supervisor - Therapy is responsible for ensuring that patient care is coordinated and managed appropriately. They serve as the first point of contact for field clinicians to ensure support is easily accessible in a timely fashion. The Clinical Supervisor-Therapy is responsible for ensuring that care and services are delivered appropriately as well as the supervision of clinical personnel. This is a leadership position that is responsible for the oversight of clinicians providing patient care to patients in their place of residence.
This is a hybrid of field work and on site based at 21HHS office:
2 Days in the Santa Rosa office (TBD)
3 days in the field: 12 patients visits each week (supervisory visits of field clinicians)

Wage range Clinical Supervisor: 143,000.00- 163,000.00 plus a generous bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match).
What you'll do as the Clinical Supervisor - Physical Therapy
Responsible for in the field oversight of clinicians who provide care and treatment to patients in their place of residence.
Daily patient activities: Handles incoming calls timely, and follows up as necessary to ensure appropriate actions are taken to meet the internal and external customer needs.
Coordinates training and educational programs for nurses within the field to improve clinical outcomes and competencies.
Assesses staff learning needs and formulates specific plans and goals for next evaluation period with employees, performs administrative call duties, and serves as a role model and resource for staff.
Functions as the first point of contact for field clinicians to assist in patient care escalations etc.
Coordinates at least bi-weekly Case Conferences with clinical team to discuss patients' progress and for revision of the care plan as needed to meet planned outcomes.
Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.
To Be a Success:
Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home care.
Possesses a degree from a baccalaureate or masters program in applicable discipline approved by an accredited organization.
Current CA Physical Therapist license
Current Basic Life Support (BLS)
Valid CA Drivers License with acceptable driving record
All license and certifications must be current at time of hire and sustained throughout employment
* You will be assigned a primary location; however, you may occasionally be asked to provide coverage outside of that area. This may include supporting vacancies, leaves of absence, higher patient census, or other client care needs.
