Job Summary: The Case Manager works under the supervision of the Administrator and is responsible for coordinating care and services for residents in the assisted living community. This role ensures that residents’ physical, emotional, and social needs are met through individualized service plans, collaboration with healthcare providers, families, and staff, and in adherence to regulatory requirements. The Case Manager serves as an advocate for residents, supporting quality of life, safety, and dignity.
Compensation: $29.00-32.00/hr; commensurate with experience and credentials.

Requirements
Typical job duties and responsibilities include but are not limited to the following:
- Support the Administrator and interdisciplinary team with implementing quality, appropriate, and cost-effective services for residents;
- Provide case management services in coordination with residents, families/representatives, healthcare providers, and other outside resources;
- Assist in enabling residents to participate in improvements in the facility and operations, and to present grievances and recommendations;
- Collaborate with marketing/sales and resident services staff to coordinate and support the admissions process;
- Participate in daily interdisciplinary team meetings and resident care conferences;
- Ensure completion and timely updates of individualized service plans, Department of Health reports, and resident personal data sheets in accordance with regulations and health status changes;
- Ensure accurate and complete recordkeeping of case management activities including notes and care plan meetings;
- Conduct regular chart audits to ensure records are compliant with NYS Department of Health regulations;
- Ensure regulatory admission processes and paperwork is completed;
- Support smooth onboarding, orientation, and adjustment for new residents;
- Provide guidance to families as they navigate care decisions, transitions, and available resources;
- Facilitate the discharge and transition-of-care process, ensuring all paperwork is completed and that residents and families experience a supportive transition;
- Establish and arrange needed services such as applications for benefits, insurance, and other services;
- Ensure residents obtain and maintain primary physician for a persons overall physical, dental, medications, and referrals;
- Collaborate with direct care staff to ensure continuity and quality of care;
- Maintain accurate, timely, and compliant documentation of assessments, service plans, and progress notes;
- Assist with audits, regulatory surveys, incident reports, and ongoing quality improvement initiatives;
- Uphold resident rights, dignity, privacy, and confidentiality in all interactions;
- Prepare and provide data or reports for risk management meetings;
- Maintain effective relationships with community-based human service agencies and represent the facility in a positive and professional manner;
- Perform other duties as assigned.

- Safety: including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs;

- Uphold resident rights as stipulated in New York State Department of Health regulations (i.e. protect and respect dignity and confidentiality of residents);
- Follow up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems, and concerns;
- Promote positive public relations with residents, family members and guests;
- Complete requirements for in-service training, acceptable attendance, uniform, and dress codes including personal hygiene, and other work duties as assigned.
Qualifications:
- Must be at least 18 years of age;
- Minimum associate degree required in Social Work, Nursing, Gerontology, Human Services, or related field from an accredited college or university with three (3) years of full-time experience; Baccalaureate or Advanced degree preferred with at least one (1) year of full-time experience in the provision of services required;
- License in Social Work or Nursing preferred;
- Prior experience of not less than two (2) years working in case management, care coordination, or a related role in assisted living, long-term care, or healthcare;
- Strong organizational skills with the ability to work independently and as part of a team;
- Excellent written and verbal communication skills, must be able to communicate clearly, concisely, and with empathy to build rapport and achieve high quality of care;
- Ability to work effectively with residents, families, staff, students, and volunteers;
- Demonstrates sound judgment, problem-solving, and decision-making abilities;
- Proficient in Microsoft Office products and experienced with Electronic Medical Records (EMR/EMAR) systems;
- Basic knowledge of resident rights;
- Must complete basic First Aid, Heimlich, and Bloodborne Pathogen training within three (3) months of hire and maintain certifications;
- Certificate of immunization against rubella required;
- Certificate of immunization against measles required if born after 1/1/1957;
- Satisfactory completion of post-offer, pre-employment physical (including a tuberculin skin test or blood assay) required;
- Appointment subject to NYS DOH approval.
