We are currently hiring Licensed Practical Nurses (LPNs) to deliver exceptional care through flexible 8-12 hour shifts. Whether you're an experienced professional or a recent graduate ready to make a difference, there’s a place for you here.

 

Licensed Practical Nurse (LPN) provides nursing care to clients in the home setting. Observe and assess the client and/or caregiver to enhance the quality of life. Demonstrate individualized creativity in educating the client and/or caregiver. Follow nursing policy and procedure per agency standards. Follow the plan of care according to physician orders.

 

Qualifications:

  • Currently licensed in the state of Florida.
  • Evidence of satisfactory Level 2 Background screening.
  • Must have physical abilities to adequately perform described duties.
  • Demonstrates adequate knowledge regarding exposure and safe handling of body fluids, and chemicals, and some hazardous equipment.
  • Demonstrates assessment skills.
  • Demonstrates ability to communicate with patients, physicians, and staff.
  • Demonstrates motivation to increase knowledge regarding home health care
  • Physical Requirements
  • Able to speak, read and write in English.
  • Able read assignments and follow directions.
  • Able to communicate and respond clear1y on telephone and respond to patient's spoken needs.
  • Able to see and hear adequately to respond to auditory and visual requests which relate to the coordination of Agency activities.
  • Able to speak in a clear, concise voice to communicate direction for Agency activities and staff.
  • Able to physically transfer, lift or assist patients whose average weight is 160 pounds with or without the aid of mechanical devices.
  • Able to spend 80% of the work standing and/or moving about
  • Able to walk, climb stairs, stoop, twist, bend, and squat to perform essential job functions.
  • Able to concentrate on detail with frequent interruptions.
  • Able to follow, complete and remember daily routines and requirements.
  • Able to comprehend and utilize professional education materials
  • Able to cope with the mental and emotional stress of the position.
  • Able to learn and apply practical training, follow physician’s orders and produce accurate records of activities and services provided.

 

Responsibilities and Duties:

  • The Licensed Practical Nurse (LPN) shall provide assigned nursing care under the direction of a Registered Nurse who provides on-site supervision as needed, based upon the severity of patients’ medical condition and the nurse’s training experience.
  • Is responsible for the clinical records for their patients. The clinical records shall be submitted timely in accordance with agency policies and procedures, for each patient or client to whom they are giving care in the home or place of residence or when they assess the care being provided by non-licensed independent contractors, pursuant to s. 400.506(13) (b), F.S.
  • Is responsible for maintaining the medical plan of treatment with clinical notes and filing the initial medical plan of treatment, any amendments to the plan, any additional order or change in orders
  • The licensed practical nurse shall be under the direction of a registered nurse, or a physician licensed pursuant to Florida Statutes, as required under 464.003(3)(b), F.S.
  • Assists the MD and RN in performing specialized procedures.
  • Prepare equipment and materials for treatment using sterile and aseptic techniques.
  • Assist patients in learning self-care techniques.
  • Report any changes in patient’s condition to the registered nurse or physician with supporting documentation.
  • Administers treatments and medications to the patient in the home setting.
  • Participates in multidisciplinary approach to patient care.
  • Maintains excellent communication with patients and staff.
  • Participate in In-Service, workshops, seminars, and self-study courses annually.
  • Maintains confidentiality of all patient and Agency matters. Ensure HIPAA guidelines and procedures are maintained.
  • Participates in discharge planning under registered nurse supervision.
  • Other duties, as assigned

 

ENVIRONMENTAL ELEMENTS
 Work is performed in patients’ homes, which may vary from clean and well-maintained to cluttered or less sanitary environments. Conditions may also vary in temperature and comfort.
