Contract/Hours: Permanent / Part Time – 28 hours per week

A bit about the role
The Action for Children Clackmannanshire Families Support Service offers a compassionate and proactive approach. This service focuses on early intervention, aiming to improve outcomes for children who may otherwise struggle to reach their full potential. Through initiatives like Homework Clubs, Seasons for Growth groups for children coping with grief and loss, and tailored one-on-one support, the team provides a nurturing environment for both children and their families. Practitioners work collaboratively with Social Work, Education, Health Professionals, and local Third Sector Organisations to ensure a holistic support network.
As a Family Support Practitioner, you’ll play a vital role in helping families adopt healthier lifestyles, equipping parents with the confidence and knowledge to foster good nutrition and wellbeing in their children. You’d encourage engagement from families at risk of diet-related health inequalities, offering both group and individual support packages that are outcome-focused and tailored to each family's needs. By working in a person-centred way, you’d help improve children's wellbeing, boost family confidence, and raise awareness around healthy living—making a lasting difference in the community.
How you'll help to create brighter futures
Some key responsibilities of a Family Support Practitioner role are;
- Having the skills, confidence and knowledge to support children and families to eat well and have a healthy weight.
- Educating parents to feel better equipped and confident to provide their children with a healthy start, including good nutrition and healthy eating habits.
- Encouraging more families at risk of diet-related health inequalities to engaging with services and accessing support.
- Providing a range of outcomes focussed group and individual support packages to vulnerable families.
- Working in partnership in a person centred way with children and families in order to improve outcomes regarding their wellbeing, confidence, ability and awareness.
Let's talk about you
- Experience of Working within a multi-agency social care, early years or health environment and applying safeguarding procedures.
- Experience of managing cases, in accordance with agreed procedures and completing assessments, planning and reviews to ensure positive outcomes.
- Working knowledge of an outcome-based approach to planning, delivering and reviewing services.
- Assessment, Service Planning and Delivery experience.
- A driving license and access to a vehicle is desirable.
