Registered Nurse - Home Health & Hospice en Grand Lake Health System

Hours of Job

• PT - 8 a.m. - 4:30 p.m. Monday - Friday

• Every 4th-5th Weekend Coverage

• 2-3 weekday nights/month (On-call coverage)

• 2-3 Holidays per year (On-call coverage)

Duties and Key Responsibilities

• Responsible for the delivery of patient care through the Nursing Process of assessment, planning, implementation, and evaluation.

• Directs and guides patient teaching and activities of Nursing Ancillary Personnel.

• Assumes a lead role in patient teaching.

• Places the patient at the center of care practices.

• Collaborates and interacts positively with the interdisciplinary team to provide quality care and achieve desired outcomes.

• May be required to work other patient care units or shifts according to the distribution of staff, patients, and/or patient acuity. 

Requirements

• Flexibility in scheduling to meet department needs.

Education/Certifications

• Graduate of an accredited school of nursing.

• Licensed RN.

Experience

• Previous experience is preferred but not required.
