Hands of Heartland is looking for compassionate and caring individuals to work with the people we support as a Supported Family Living Direct Support Professional (DSP).
What You’ll Do:
As a Supported Family Living DSP, you will work directly with people with developmental disabilities assisting them with day to day living activities. You will support individuals with various activities in a home environment and in community settings including meal planning, grocery shopping, or household chores.
Key Responsibilities:
Assist with activities of daily living (ADLs) such as hygiene, dressing, mobility, and meal preparation, based on the individual’s level of need.
Support the individual in developing and maintaining skills for independent living, which could include money management, household chores, self-advocacy, and communication. Based on individual's needs, goals, and support plan.
Implement goals and objectives from the individual’s person-centered support plan and document progress as required.
Monitor and promote the individual’s health and well-being while following safety protocols.
Encourage social interaction and community inclusion through meaningful activities and relationship building.
Maintain respectful, professional, and confidential communication with the individual, their family, and the interdisciplinary support team.
Comply with all applicable policies, procedures, and state regulations related to waiver services.
This position may require you to transport individuals in company vehicles. You will be paid extra for driving.
Shifts Needed: Day and Evening hours.

Requirements: Must be 18 years old. Must have a valid driver’s license and car insurance. Must be able to pass all background checks. Experience not required.
Pay: $16 an hour. We pay more for experience and if you have a CNA license.
