Description
Purpose: The Registered Nurse (RN) at the Wellness Center is responsible for delivering patient-centered care, managing wellness screenings, supporting provider services (including virtual care), participating in community outreach events, and completing assigned tasks at other WWFHC locations as needed. The RN ensures compliance with regulatory requirements, maintains accurate data tracking, and fosters strong community relationships by promoting preventive health and wellness.
PART I: ESSENTIAL POSITION FUNCTION AND DUTIES
Key Responsibilities:
- Conduct all required wellness screenings, including blood sugar, Diabetes HbA1C, total cholesterol, HDL/LDL/triglycerides, point-of-care (POC) COVID-19 antigen tests, and distribution of OTC test kits. Hypertension diagnosis and management will be done. Taking proper blood pressure readings and understanding when to send a patient to the ER for a hypertensive emergency/crisis and proper follow up for elevated readings is crucial.
- Collect, track, and submit screening data to the Michigan Department of Health and Human Services (MDHHS) in compliance with reporting requirements.
- Register clients in WWFHC’s electronic medical record (EMR) system when they are seen by a provider through telehealth.
- Calibrate screening devices, manage client flow, and ensure accurate, timely documentation of results.
- Print and provide screening results to clients, ensuring they understand findings and next steps.
- Connect clients with virtual providers and support consult coordination as needed.
- Based on screening results, refers clients to be seen by a provider at WWFHC for further evaluation and care.
- Oversee clinic setup, unpack and assemble equipment, order supplies, monitor inventory, and troubleshoot technical or operational issues.
- Provide client education on health promotion, chronic disease prevention, and care plan reinforcement.
- Collaborate with multidisciplinary teams to ensure seamless care delivery and positive outcomes.
- Participate in community outreach events, providing wellness screenings, education, and support to expand access to care and promote preventive health initiatives.
- Complete required end-of-day reporting to ensure data accuracy, supply tracking, and operational compliance.
- On days not assigned to the Wellness Center, complete tasks and assignments at other WWFHC locations as directed.
- Uphold compliance with organizational policies, confidentiality requirements, and all federal and state regulations.
- Assist in training, mentoring, and orienting Medical Assistants or support staff.
- Perform other duties as assigned to support wellness center operations, community health initiatives, and outreach programs.

- Current license as a Registered Nurse required
- Current Basic Life Support certification required.

Qualifications
