Description
UCFS is looking for candidates who are passionate about making a difference in the lives of others! We are currently seeking a full-time (40hr) Certified Dietitian/Nutritionist to provide expert nutritional guidance and support to our patients, helping them achieve their health and wellness goals through personalized nutrition plans and education, including but not restricted to diabetes and obesity.
Essential Responsibilities:
- Provide medical nutrition therapy as required based on treatment plan developed by the clinical provider
- Develop and implement nutritional strategies based on patient uniqueness and individual health history circumstances
- Provide nutritional education in-service training to other members of the clinical care teams.
- Participate in community events as needed to provide nutrition education and health programs.
- Maintain best practice knowledge level by participation in continuing education opportunities, and utilization of current professional reference materials; maintain continuing education requirements as needed for licensure and board certification.

Requirements
- Certified Nutritionist: either a Certified Nutrition Specialist/ Certified Clinical Nutritionist (CNS/CCN)
- Certified by Department of Public Health to practice in the State of Connecticut
- Must enroll with the Connecticut Medical Assistance Program (CMAP)/ Husky Health
- Preferred 5 years of experience preferred with a strong community health/public health orientation and patient care management experience
- Preferred knowledge of using Epic EMR.
