This position is FT, 36 hours per week, every 4th weekend and a holiday rotation.
Hours are either 9a-930pm, or 10-1030pm
Summary of Position: The Admissions Nurse is responsible for enrolling patients in the hospice programs and assisting with the referral and intake process.
Education and Experience:
- Graduation from an approved School of Nursing.
- Current RN license in the State of Ohio.
- Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing.
- Hospice experience preferred.
Special Skills and Requirements:
- Shall have a valid driver’s license.
- Excellent customer service skills.
- Detail oriented with excellent communication, documentation, and organizational skills.
- Ability to follow instructions.
- Ability to work independently without supervision.
- Travel to other sites as needed. Travel may be in inclement weather.
- Ability to lift 25 lbs of supplies or equipment.
- Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking. Ability to assist with patient care activities such as ambulation, transfers, and positioning.
Responsibilities of Position:
- Explain hospice services and benefits in regards to Medicare, Medicaid and Private Insurances and knows when to use Palliative Care Interventions’ and ABN’s.
- Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
- Order appropriate Durable Medical Equipment (DME) for patient.
- Perform competent initial nursing assessments.
- Provide competent nursing care to assigned patient through skilled symptom management, emotional support, appropriate teaching and timely response to patient and family needs.
- In the absence of admissions and referrals, the admission nurse is responsible for the following:
- Care Center Assistance
- End of Life/Continuous Care
- Routine Homecare Visits
- Chart Reviews
- Other duties as assigned
- Work evenings and weekends as needed to meet the needs of the patients and families.
- Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
- Other duties as assigned.
