Position Summary
The Licensed Practical Nurses (LPN) working in the Horizon Clinics are responsible for the overall medical care of assigned adult clients, including the service delivery of basic nursing assessments; delivery of client/family education and diagnostic procedures; consultation with physicians; and carrying out of doctor's orders.
Required Education and Experience
- The successful completion of a course of study that allows for licensure as a Licensed Practical Nurse in the state of VA is required.
- One year of experience providing nursing services as an LPN is required or an equivalent combination of education and experience that provides the required knowledge, skills, and abilities.
Related Knowledge, Skills and Abilities
Knowledge of community mental health and public health to provide advocacy and enhance an individual’s treatment and access to community resources. Knowledge of nursing best practices. Knowledge of the principles, procedures, techniques, trends, and literature related to mental health, physical health, intellectual disabilities and substance abuse. Knowledge of psychiatric/medical medications/treatments.
Willing and available to work modified schedules (weekends, holidays, etc.) as required. Ability to drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, and provide services to clients at other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites; the ability to participate in and successfully pass behavioral intervention training, including both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.
