Position Title: Long Term Substitute Needed for Special Education Teacher/Transition Specialist
*Will Consider PPS Staff for this position
QUALIFICATIONS:
1. Current Connecticut certification as a special education teacher, counseling, or social worker certification, etc.
2. Knowledge of transition services requirements per federal and state regulations.
3. Strong interpersonal skills and ability to form collaborative relationships within the community
4. High degree of initiative and ability to work independently
REPORTS TO: Director of Special Education and Pupil Services and Supervisor of Special Education (6-12+)
JOB GOAL:
The special education transition specialist plans, implements, and evaluates transition related services to students under the Individuals with Disabilities Education Act. This individual will form collaborative relationships within the community in order to provide students with a variety of vocational opportunities as part of the district’s Community Partnership Program.
PERFORMANCE RESPONSIBILITIES:
Assesses students’ interests, skills, and abilities related to post-secondary education, employment, independent living, and community participation using a variety of transitional tools and assessments
Plans, implements and evaluates transition related activities for students with disabilities both in the high school and community settings
Maintains an effective community-based training program for students to provide them with relevant and functional experiences.
Serves as a liaison between the district’s Planning and Placement Team and the representatives of public and private adult service agencies.
Seeks and maintains cooperative relationships with community, regional, and state resources and agencies to support transitioning students and their parents.
Provides consultation services to students, parents, and staff in transition-related matters.
Establishes school and business partnerships to support students’ participation in community-based work experiences.
Arranges for a variety of student experiences in accessing the community, including recreation and leisure skills and independent living skills for students preparing to exit high school as well as students in the Community Partnership Program.
Develops appropriate transition goals with staff, students, and parents.
Plans and coordinates job shadowing experiences for students.
Meets regularly with job coaches and paraeducators and provides continuous training, feedback, support, and oversight to them.
Coordinates the special education work study and internship program. Monitors and evaluates student performance in the work study program.
Identifies transition-related needs in the district and communicates them to the Supervisor of Special Education and Director of Special Education and Pupil Services.
Performs other duties as may be assigned by the Director of Special Education and Pupil Services.
