The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.

Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. The NCSB is seeking a Case Manager III in their Housing and Homeless division. The Housing and Homeless division serves individuals experiencing homelessness in Norfolk through a variety of programs including outreach, housing planning, emergency shelter, and supportive housing programs. 

The Case Manager III will provide time-limited wraparound services to persons experiencing homelessness/hospitalization with behavioral health disorders as they transition into housing. Completes housing, treatment. and services plans that are consumer-directed. Works to activate the services needed for and preferred by the individual to provide behavioral health services and treatment: medical care; benefits access employment or vocational services: and housing acquisition. Links clients to services and supports in the community while facilitating the client's transition out of CTI services and into mainstream supports. Service connections cover the City of Norfolk including working with providers in those communities to connect persons to care. Completes billable case management services to those eligible.

Department Hiring Salary Range: $46,589-$53,577

Position Duties

Essential functions include but are not limited to:

  • Active outreach to engage persons identified by hospital discharge planners, outreach workers, and other referral sources who qualify for the State funded Permanent Supportive Housing Program, follow-up for effective engagement, and follow-through with making connections to services, resources, treatment, and housing.
  • Provide time-limited, wraparounds support and services beginning at the time of hospitalization/homelessness and lasting approximately nine months while working to link clients to mainstream supports in the community.
  • Keep and submit accurate documentation of activities, including the collection and documentation of required information.
  • Provide activities to overcome access barriers to services, resources, and housing.
  • Assist with the development of applications for benefits and housing programs.
  • Provide services in a person-centered environment, utilizing best practices and methods as directed by the CTI framework.
  • Work in the field and in the office, plus providing services in community settings such as shelters, streets, public spaces, treatment programs, and benefits offices. Provides follow-along services to persons once housed to effectively transition them to those providing ongoing supports.
  • Provide development of services planning that addresses treatment, medical, shelter, benefits, and housing planning based on the person's needs and preferences. Work effectively with rest of team to ensure ongoing work with consumer on the plan.

Minimum Qualifications

Bachelor's degree in a human services field. 

Two years' experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations.

Preferred: 
  • Experience working with people experiencing homelessness and behavioral health disorders.
  • Experience assisting people in gaining housing.

Other Qualifications

  • Valid Driver's License may be required depending on assignment.
  •  Certification in basic first aid and CPR, depending on area of assignment.
  • QMHP-A preferred.
Working Hours: Monday-Friday 8am-4:30pm
