Hours: Monday – Friday 10a-6p Flex (40/hrs.)
Starting Pay: $19.50/hr.

You’ll lead and mentor teams, coordinate daily support efforts, and build strong relationships that uplift the people we serve, making a meaningful difference every single day.
As a Supported Living Manager-1 at Pathfinder, You Will:
Plan and Coordinate Daily Support
- Schedule and ensure staffing coverage for your assigned residential site
- Arrange medical appointments and coordinate transportation
- Support individuals with financial and banking tasks
- Maintain medication protocols and ensure compliance with care plans
Support and Mentor Your Team
- Train and supervise Direct Support Professionals (DSPs)
- Conduct regular team meetings and performance reviews
- Approve timesheets and provide hands-on coverage when needed
- Promote positive communication and teamwork
Ensure Documentation and Compliance
- Track Individual Support Plans (ISPs) and enter goals into Provide
- Complete and upload quarterly summaries to the DDRS Portal
- Maintain Medicaid Waiver files and complete all required forms
- Serve as the main point of contact for the Interdisciplinary Team (IDT), guardians, and external partners
Train, Advocate, and Empower
- Use person-centered thinking to guide every interaction
- Train DSPs in orientation, ISP objectives, and behavior support plans
- Participate in 6-month and annual IDT meetings
- Encourage personal growth, community inclusion, and self-advocacy among individuals served

Requirements
Preferred Minimum Qualifications
- Minimum of a high school diploma/GED.
- Prefer 2 years experience in the field of Human Services.
- At least 18 years of age.
- Basic language, written, and computer skills necessary for communication and documentation.
- Demonstrated ability to support people with disabilities.
- Ability to work a flexible schedule if needed.
- Crisis prevention, intervention, and teaching skills.
- Valid driver’s license, insured vehicle, and safe driving record.
- Ability to speak, hear, and read the English language.
