About the Role

The Aged Care On-Site pharmacist (ACOP) is a credentialed pharmacist who works within our residential aged care facilities (RACF). The ACOP will bring their expertise in medication management and pharmacotherapy to provide collaborative and integrated care for all residents as part of the multidisciplinary healthcare team at IRT. 

Reporting to the Operations Manager, you will develop a collaborative working relationship and integrate within the team, both internally and externally. You will provide your expertise with a focus on person centred care,work closely with GPs, on- site nurses, facility staff and other health care professionals. Your role will aim to improve health outcomes of residents and build confidence with families, whilst also providing expertise in medication management to reduce medication related harm and optimise medicines use.

 

Our Berala on the Park (Auburn) aged care centre is looking for an on-site pharmacist to work in a Part Time capacity across 3 days a week.

 

Your work may include:

  • Being readily available to aged care staff and residents, building collaboration with the health care team, including general practitioners and community pharmacies
  • Provide continuity in medication management, such as day-to-day monitoring of residents’ medication and solving medication related issues 
  • Assisting with medication management and communication during transitions of care 
  • Undertaking whole of facility quality use of medicines activities, such as medicine use evaluations and implementation of changes identified in audits to improve the use of psychotropics, antimicrobials and other high-risk medications 
  • Being readily available and a central contact to address medication management enquiries from residents, families and carers, RACF staff, medical practitioners (GPs and specialists), community and hospital pharmacists and other health professionals.
  • Providing culturally safe services.
  • Ensuring the resident and family are well informed and share the decision making for medication related decisions.
  • Participation in GP rounds

To Be Successful You Will Have

  • Registration as a pharmacist with a valid Australian Health Practitioner Regulatory Agency (AHPRA) number
  • to be credentialed having completed an APC accredited ACOP training program
  • MMR credentialed pharmacists participating in the Measure will need to complete an APC accredited ACOP training program or recognition of prior learning prior to 30 June 2026
