We are seeking a Hospice RN Care Manager to join our team at Independent Living Systems (ILS). About the Role:
The Hospice RN Care Manager plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses. This position is responsible for coordinating and managing comprehensive hospice care plans that address the physical, emotional, and psychosocial needs of patients and their families. The Hospice RN Care Manager collaborates closely with interdisciplinary teams, including physicians, social workers, chaplains, and aides, to ensure seamless communication and continuity of care. By conducting thorough assessments and ongoing evaluations, the Hospice RN Care Manager ensures that care is tailored to each patient’s unique condition and goals. Ultimately, this role aims to enhance quality of life, provide symptom management, and support patients and families through the end-of-life journey with dignity and respect.
Minimum Qualifications:
- Current and valid Registered Nurse (RN) license in the state of Florida.
- Minimum of two years of clinical nursing experience, preferably in hospice, palliative care, or related healthcare settings.
- Basic Life Support (BLS) certification.
- Strong knowledge of hospice care principles, symptom management, and end-of-life care standards.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
Preferred Qualifications:
- Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
- Experience with electronic medical records (EMR) systems and hospice documentation software.
- Familiarity with Medicare hospice regulations and reimbursement processes.
- Advanced training in pain and symptom management or psychosocial support.
- Previous experience in case management or care coordination roles within hospice or home health environments.
Responsibilities:
- Conduct comprehensive patient assessments to develop individualized hospice care plans.
- Coordinate and oversee the delivery of nursing care and symptom management in collaboration with interdisciplinary team members.
- Provide education and support to patients and their families regarding disease progression, care options, and advance directives.
- Monitor patient status regularly and adjust care plans as needed to respond to changing clinical conditions.
- Document all patient interactions, care plans, and clinical updates accurately and timely in compliance with regulatory standards.
- Serve as a liaison between patients, families, healthcare providers, and community resources to facilitate holistic care.
- Ensure compliance with hospice policies, procedures, and regulatory requirements while maintaining patient confidentiality.
- Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient outcomes.
