Essential Functions:
• Works collaboratively with Director of Pharmacy, Pharmacy Leads, 340B Compliance and Quality Manager, and interdisciplinary teams to identify opportunities for new or expanding clinical initiatives (i.e. Chronic Disease Management, Meds to Beds, Transition of Care, Specialty medications, collaborative practice agreements, ambulatory pharmacy integration).
• Design, pilots, and implement clinical initiatives with measurable KPIs and impact on quality, safety, patient satisfaction, revenue expansion and or cost reduction.
• Leads and or support grant-funded and or valued-based care initiatives involving pharmacy services.
• Collaborate with pharmacy staff to identify workflow improvements and system enhancements.
• Supports pharmacy formulary optimization, guidelines development and clinical decision support tools in partnership with P&T Committee, Quality, and Behavioral Health teams.
• Perform data analysis to assess trends, identify gaps, and recommend process improvements.
• Precept pharmacy students and residents on innovation-focused rotations and projects.
• Assist in the onboarding and training of pharmacy staff on system functionality and updates. Works collaboratively with vendors and IT to enhance clinical EMRs and functions with pharmacy OS
• Ensure clinical innovation initiatives align with District’s mission, organizational goals and objectives as well as payer initiatives and state/federal regulatory guidelines.
• Create and maintain technical documentation, clinical workflow diagrams, and standard operating procedures (SOPs) on new and or improved clinical initiatives.
Other assigned tasks as requested, including but not limited to pharmacy coverage as business needs demands.

Qualifications
Education:
PharmD from an ACPE- accredited program.
Experience:
• Experience in retail or ambulatory pharmacy operations. Commitment to continuous learning and practice advancement. Interdisciplinary team collaboration.
• Familiarity with EMR systems (i.e. Epic) and pharmacy operating processing systems (i.e Rx Enterprise)
• Strong communication skills, problem solving and project management skills. Outcome driven and data-informed decision making.

Active Florida Registered Pharmacist License in good standing
