THE OPPORTUNITY

Feeding Westchester has an exciting contract opportunity for a mission-driven Neighbor Outreach and Data Coordinator. Reporting directly to the Program Associate, the Neighbor Outreach and Data Coordinator, support Feeding Westchester’s participation in the 1115 Medicaid Waiver program administered by Hudson Valley Care Coalition/Social Care Network as well as the client data system used by our 175+ partners called Link2Feed. We are looking for a candidate that is personable, especially over the phone and shows a high attention to detail.

 

The 1115 Medicaid Waiver program provides Medically Tailored Food Boxes to Medicaid patients who are screened and determined eligible for the service. The Coordinator will oversee Feeding Westchester’s workflow for this initiative, ensuring compliance, timely execution, and accurate reporting. They will manage the internal operations of accepting referrals, coordinating with the operations department to ensure orders are picked and delivered on time and then will bill for these services using the Unite Us platform.  We are expecting to grow this program to 200 - 300 clients / deliveries per week.    The coordinator will also get screened to become a 1115 Medicaid Waiver “Screener” and “navigator”, which are both reimbursable services that will help get more Medicaid patients screened and referred for services. 
 
Link2Feed (often shortened as L2F) is a cloud-based client intake and case management software designed specifically for food banks, pantries, and social service organizations. The coordinator will manage back-end support for the organization’s Link2Feed data system, maintaining smooth operations and serving as a resource to resolve issues for staff and partner agencies.  They will serve as a link between pantries and the Link 2 Feed organization and will lead on uploading new clients into the system when a new partner is onboarded to Link 2 Feed. 

 

 

ESSENTIAL FUNCTIONS & RESPONSIBILITIES

1115 Medicaid Waiver Program:

  • Coordinate Feeding Westchester’s workflow under the 1115 Medicaid Waiver program.
  • Track referrals, eligibility, and distribution of Medically Tailored Food Boxes to Medicaid patients.
  • Ensure compliance with Hudson Valley Care Coalition/Social Care Network program requirements.
  • Maintain program documentation, reporting, and data integrity.
  • Bill for services rendered
  • Serve as liaison with program partners and Feeding Westchester staff to support program delivery.


Link2Feed (L2F) Data Management:

  • Manage the execution of Link2Feed Memorandum of Understanding (MOU) agreements.
  • Submit support requests and troubleshoot issues in real time with L2F.
  • Maintain user accounts, access rights, and permissions.
  • Schedule and coordinate quarterly L2F trainings for staff and partner agencies.
  • Import and clean client data from spreadsheets into Excel/Link2Feed.
  • Serve as point of contact to resolve issues between pantries/agencies and the L2F system.


Reporting & Compliance

  • Maintain accurate and timely records for both Medicaid Waiver and Link2Feed program areas.
  • Identify barriers or process improvements to strengthen program efficiency and compliance.

  

POSITION REQUIREMENTS

 

Required Education/Experience

  • Bachelor’s degree or equivalent combination of education and relevant work experience.
  • Minimum of 2–3 years’ experience in data management, program administration, or healthcare/social service coordination.
  • Experience working with Medicaid, healthcare, or hunger-relief programs preferred.
  • Familiarity with client data management systems 


Required Knowledge/Skills

  • Strong organizational skills with attention to detail.
  • Proficiency in Microsoft Excel, including imports and data management.
  • Excellent verbal and written communication skills.
  • Problem-solving and troubleshooting abilities with databases and technology.
  • Ability to manage multiple priorities independently and meet deadlines.
  • Capacity to build strong, collaborative relationships with staff and external partners.


 
Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to communicate regularly by email, phone, and in person.


 
Working Conditions

  • Based primarily in Feeding Westchester’s offices, with potential for hybrid/remote flexibility.
  • May require occasional evening or weekend hours for program or training support.

SALARY RANGE: $35/hour
