The Hospitalist Nurse is a registered nurse who collaborates with the hospital care team and floor staff to coordinate and manage the process of patient care from the point of admission through discharge and hand off to their primary care provider.
Responsibilities:- Initiates and assists with department and internal operations to optimize patient care.
- Assess patients as to their current clinical status and to gathering formation such as outpatient medications, laboratory results and prior medical records in preparation for the physician assessment, observation and inpatient status, who have perceived change in status or acuity require further evaluation.
- Providers leadership through communication about and validation of patient care.
- Assist in enhancing workflow efficiencies by proactively contacting various departments.
- Conduct an afternoon touch base with Hospitalist team and specialists to discuss potential discharges for next day in order to facilitate an early discharge.
- Assist in enhancing the quality of care.
- Assist in optimizing resource utilization and reducing the risk of hospital acquired conditions.
- Work collaboratively with Hospitalists to enhance quality improvement efforts.
- Working directly with Hospitalist team by identifying readmissions that could have been prevented and develop a targeted plan of care with care team to prevent future rehospitalizations of such patients.
- Post discharge follow up with patient to provide update on any new results and change in plan of care based on additional information.
- Maintenance of Epic in basket.
- Accurately relay the problems efficiently to the covering physician.
- Evaluates patient progression toward outcomes.
- Educating and addressing the patient, family members and RN on the patient needs.
Certifications: Basic Life Support (BLS) within 30 days - American Heart Association (AHA); Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR), Education: Bachelor's Degree: Nursing, Work Experience: 3+ years of Acute Care Nursing

Salary Range:The compensation range for this position is $32.81per hour - $56.43per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
