JOB SUMMARY

The Senior Case Manager works closely with and supports the Lead Case Manager.  The SCM will ensure that services are individualized, appropriate and documented according to ORR requirements, state licensing standards, and Abbott House’s policies/procedures. Responsible for case management including admission, service planning, reunification, discharge planning, and documentation including completion of the Family Review Process, Case Summary and review of ISP and all reunification coordination with ORR.

GENERAL RESPONSIBILITIES

  • Will work closely and support the Lead Case Manager.  Will cover for the Lead Case Manager whenever she is out. Ensures a timely and safe unification.
  • Works with the Lead to run and review the weekly Tableau Reports.  Reviews cases expeditiously to submit for discharge.
  •  Co Leads group supervision.
  • Carry a case load (1:4) ratio. Covers cases for case managers when they are out.
  • Demonstrates the ability to assess each case and adapt service plan throughout the life of the case.
  • Documents all contacts appropriately in case record and in the UC Portal. Writes timely, clear and concise progress notes and other documents as required.
  • Screens for human trafficking concerns.  Completes necessary assessments, identifies sponsor, assures background checks are completed, maintains contact with the sponsor.
  • Help young ones identify and screen prospective sponsors. Manages reunification process.
  • Establishes effective collegial relationship with Site Supervisor.
  • Effectively coordinates inter and intra-agency services to clients.
  • Attends meetings and otherwise monitors the provision and documentation of educational services.
  • Makes timely and appropriate referrals for medical services, health screening and mental health assessment of young people.
  • Makes meaningful contributions to agency, departmental and unit programming.
  • Responds to client emergencies and case management 24/7. 
  • Develops sound working relationships with staff of outside agencies.
  •  Demonstrates the ability to carry out the above responsibilities in a professional manner.
  • Attend and complete all mandatory ORR & Abbott House training on a timely basis.
  • Any other related duties as required.

  • Computer – The Senior Case Manager must be able to use a computer and able to learn software programs
  • Office Equipment- The Senior Case Manager is required to operate a fax, copier, scan, and other office equipment.
  • Communication - The Senior Case Manager needs to be able to speak, read and write fluently in the English language
  • Writing - The Senior Case Manager must be able to write in the English language using correct spelling, grammar, punctuation, etc.
  • Lifting - The Senior Case Manager may frequently lift 0-25 lbs., occasionally lift 25-50 lbs., and rarely lift over 50 lbs.
  • Push/Pull - The Senior Case Manager may frequently push/pull 0-25 lbs. of force, occasionally push/pull 26-45 lbs. force, and rarely push/pull over 45 lbs.
  • Sitting, Standing and Walking The Senior Case Manager will be required to sit, stand and walk throughout the work day.
  • Stooping -The Senior Case Manager may occasionally stoop.  This is defined as bending body downward by bending spine at the waist.
  • Kneeling -The Senior Case Manager may occasionally kneel.  This is defined as bending legs at knees to come to rest on knee(s).
  • Crouching - The Senior Case Manager may occasionally crouch.  This is defined as bending body downward and forward by bending spine and legs.
  • Work environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee frequently works in an office environment. The noise level in the work environment is usually moderate due to talk and movement of the program participants and other co- workers.  The noise level can increase on occasion due to yelling or screaming by the participants of the programs.

Qualifications


QUALIFICATIONS

  • BSW or BA/BS in human services required + experience in child welfare/child protective services and at least 3 years’ experience working with UC program.  
  • Excellent writing and communication skills including proficiency both written and oral in Spanish and English.
  • Candidate must have demonstrated ability to reach out and work with community resources and foster parents.
