The role you’ll contribute:
Responsible for conducting individualized needs assessments, planning and providing individualized and group instruction in diabetes self-management education/training and meal planning and Medical Nutrition Therapy.
The value you’ll bring to the team:
· Responsible for conducting individual assessments and other aspects of diabetes self-management skill based on the needs assessment.
· Documents in the permanent education record all aspects of the education services provided.
· Provides and documents follow-up for patients who receive education.
· Assists in planning, implementation and evaluation of the diabetes patient education program.
· Interacts with other health professionals as a productive team member
· Promotes the diabetes center services through participation in community activities
· Accountable for assuring the diabetes diet instruction complies with national standards.
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
Minimum of a Bachelor’s degree with a major in Food / Nutrition required.
Registered by the Commission on Dietetic Registration of the American Dietetic Association required.
Licensed by the Texas State Board of Examiners of Dietitians preferred.
National certification as a Diabetes Educator (CDE) preferred.
