Conduct initial participant intake and connect them with appropriate preventative services.
Manage a caseload of clients, providing ongoing support and case documentation.
Facilitate community education and outreach campaigns on program focus areas.
Collaborate with the Registered Dietician to ensure participants have access to nutritional support.
Monitor participant progress and address barriers to care, such as transportation or financial challenges.
Track referrals to healthcare providers and measure outcomes for program reporting.
Develop and maintain relationships with community partners to enhance participant support.
Assist participants in navigating healthcare systems and accessing additional resources.
Coordinate follow-ups with participants to ensure continuity of care and engagement in services.
Provide feedback to the SIRHSO Assistant Director on program challenges and opportunities for improvement.
- Provide critical support services to program participants, ensuring active engagement with grant activities and working to address barriers to successful outcomes.
- Assist with community education and communication campaigns related to specific special focus areas, including heart disease, cancer, chronic respiratory disease, and unintentional injuries.
