This position is responsible for responding to referrals from physicians, hospitals, SNFs, ALRs and the community with skills to include exceptional customer service, performing data entry, retrieval information from various software systems, documenting, processing and following up on hospice referrals. The Admissions Clinical/ Intake Coordinator serves as a customer service liaison with referral sources to obtain documentation to support terminal diagnosis and admission to hospice.
Qualifications
- Minimum of three (3) years training or equivalent work experience in clerical, administrative or billing services preferred.
- Must possess a minimum of intermediate computer and software skills.
- Netsmart EMR knowledge preferred.
- This position also requires strong communication skills including excellent verbal and writing abilities.
- Minimum Associates degree preferred.
- This position also requires an understanding of end of life care, and must have the ability to maintain harmonious, constructive relationships with internal and external customers.

- Responsible for securing approvals in community and nursing home applications.
- Coordination with the community and facilities to meet their service needs.

- Demonstrate effective written and verbal communication skills. Provide information to prospective patient, families, and referral sources. Demonstrate effective coordination of referrals from physicians and other referral sources.
- Shares in staffing coverage when necessary on weekend/holiday and emergencies.
- Participates in agency and community programs as requested to promote professional growth and understanding of hospice care.
- Data entry of information.
- Maintain a high level of urgency, customer consciousness and service excellence.
- Participate as an active supporter of the Treasure Coast Hospice compliance program, including an emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization’s compliance program.
- Teach and uphold Treasure Coast Hospice’s standards with a focus on perfect documentation, perfect visits, perfect communication, and professional dress & behavior.

Position will be categorized as category III (involves no routine or potential exposure to blood, body fluids/tissues).
Working Conditions
- Typing or otherwise working primarily with fingers rather than with the whole hand as in handling.
- Sedentary – Exerting up to ten pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria met.
- The work is required to have close visual acuity such as/preparing and analyzing data and figures; transcribing, viewing a computer terminal; extensive reading.

Incumbent may be required to perform emergency duty before, during and/or beyond normal work hours or days in the event of an emergency, crisis situation or disaster (man-made or natural) including evacuation sites. The person in the position will work in a smoke-free location, and is expected to adhere to all smoking restrictions.
