Case Manager Job Description 

 

PAY SCALE: $27.00 to $32.91 per hour. 

FTE: 1.0 FTE (40 hours per week).  

Hours and Location: Monday - Friday, 9:00AM - 5:00PM This position is out in the field and in all types of weather conditions.

DEPARTMENT: EVOLVE Peer Delivered Services 

 

Job Description: Central City Concern (CCC), Transition Projects Inc. (TPI) and THE PEER COMPANY are partnering to field a Navigation Team which conducts direct street outreach to people living in unsanctioned encampments. As a part of this multi-disciplinary Navigation Team the Case Manager (PCM) will provide equitable access to the coordinated access system, assess and triage people for housing and other resources, and build linkages to under-served communities. This dynamic position is highly mobile across the county and requires strong coordination and engagement skills. This is not a peer support position but is case management informed by lived experience. 

The PCM makes initial contact with people living outdoors. Through a combination of referral information, assessment and meeting with the person being served, the PCM will explore what services and supports are needed and desired. The PCM will work in close collaboration with the Navigation Team Manager, Peer Support Specialist (PSS) and team members from CCC and TPI. The PCM assists the Evolve Director and the Navigation team to ensure activities are in agreement with JOHS deliverables. A candidate with CADC I credential is highly valued. 

Duties and Responsibilities:  

  • Provide outreach and engagement to people experiencing homelessness in a manner that focuses on safety, trauma informed approaches, and relationship building to assist people to connect to needed shelter, housing, and needed services.
  • Coordinate with community social service partners for linkage to medical, mental health, and addictions, and culturally or youth specific services when needed.
  • Administer VI-SPDAT and VAT tools for Coordinated Access prioritization.
  • Support individuals as needed to obtain documents necessary for housing placement. 
  • Participate in Coordinated Access for Adults System Case Conferencing meetings as needed.
  • Create and maintain relationships and resource knowledge specific to the population being served.
  • Attend staff meetings, case management meetings and partner meetings as assigned.
  • Follow Case Management Best Practices.
  • Stay updated/current on emergent and existing community resources available to PSS’s and program participants. Locate and familiarize themselves with local resources. Serve as liaison for resource information for peer support.
  • Stay updated/current on events/issues in the broader peer support community by attending annual Peerpocalypse conference and professional development opportunities as appropriate.
  • Attending all pertinent partner meetings with Program Director or independently.
  • Input and maintain services provided in Recovery Link and HMIS databases.
  • Maintain accurate, updated, and comprehensive participant files. 
  • Maintain accurate and updated documentation of services in Recovery Link and HMIS.
  • Answer inquiries about other The Peer Company peer support programs and how to send referrals.
  • Understand data collection and all digital data collection services to ensure grant deliverables are met.
  • Communicate with all staff and supervisor in a timely fashion. 
  • Other duties as assigned. 

Qualifications for Position: Ideal candidate identifies as having lived experience of addiction, mental health challenges, and incarceration. Understands the effect of trauma on health, coping, and other aspects of individual needs. Understands the principles of recovery, consumer-involvement and trauma-informed care. 

Bachelor’s degree preferred. Case management experience required. Three to five years as a Peer Support Specialist/Certified Recovery Mentor or related experience required: an equivalent combination of education and experience will be considered sufficient. Peer Support Specialist/Certified Recovery Mentor training highly valued.  

Valid Oregon Driver’s License and proof of automobile insurance required. Reliable transportation necessary. 

Skills and Knowledge Required: Ability to network and collaborate with diverse groups of people who have varied skills and knowledge. Ability to read, analyze and interpret mental health and recovery periodicals, professional journals, and government regulations. Well-developed writing skills including the ability to write for publication, and to create reports, technical assistance guides, and educational and training materials. Graduate level education and experience in assessment. Ability to respond to questions from other community organizations, consumers, and the general public. Must be proficient with Microsoft Office Suite. Must be technologically savvy with the ability to quickly learn to navigate new data collection systems.  

Personal Qualities: The ideal candidate for this position is passionate about and committed to the The Peer Company mission. Must be a good communicator and sensitive to various communication and learning styles. Is comfortable with the shared duties and daily prioritization shifts inherent to a small non-profit environment. Must have lived experience of mental health and addiction recovery. Possesses an absolute belief in every peer’s ability to learn, grow and recover. Possesses insight pertaining to personal biases and worldview and how they may interfere with effectively working with individuals representing a variety of cultural, ethnic, language and life experiences.
