The Position: The Nurse Practitioner provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site setting. 

• Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients
• Orders and interprets diagnostic and therapeutic tests relative to patients age-specific needs
• Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities upon consultation with the patients physician
• Monitors the effectiveness of treatment and interventions
• Facilitates staff, patient and family decision making by providing educational tools
• Develop, in conjunction with other clinical team members, each patients Care Map and evaluate patients response to therapy during each home visit or encounter
• Appropriately discharge primary patients and complete discharge summary within 7 days of discharge
• Develop and maintain caseload scheduling and communications on a weekly and monthly basis in updating schedule with changes
• Document all patient care activities and interventions as required by organizational policy and procedure and law and regulation
• Coordinate care via live telephone conversations and/or voicemail with the team as determined by the urgency of information to be relayed

Schedule:

• 8:30a-5p Monday-Friday
• Full-Time 

Requirements

• Minimum 3 years nursing experience with background in infusion therapy
• Excellent IV skills
• Graduate of an accredited school of nursing
• Graduate of an accredited Nurse Practitioner Program
• Current Advanced Practice Registered Nurse (APRN) Licensure
• Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS)
• Strong computer skills, including Word, Excel, and PowerPoint
• Excellent customer service skills
