Coastal Health is seeking a compassionate, organized, and detail-oriented Case Manager to join our Quality Department. In this vital role, you will help bridge the gap between hospital discharge and follow-up care — ensuring patients receive the resources, support, and appointments they need for a smooth and safe recovery.
💡 Position Overview
As a Case Manager in our Quality Department, you will serve as the first point of contact for patients discharged from the hospital, providing timely and coordinated care support. You will play a key role in identifying unmet medical needs, managing post-discharge follow-up, and documenting all communication and care coordination within the first 48 hours.
🔹 Key Responsibilities

Contact patients within 48 hours of hospital discharge to begin follow-up process

Schedule follow-up appointments after ER visits or hospital stays (within 14 days)

Confirm post-discharge care arrangements including:

Home readiness
-
Durable Medical Equipment (DME)

Specialist referrals
-

Document and notate any medication changes or concerns during follow-up

Provide support with:
-
Scheduling or contacting Home Health Care (HHC) if patients cannot reach them

Arranging transportation for follow-up visits, when needed
-

Ensure all patient communications are thoroughly documented and accurately recorded

Collaborate with internal teams to close care gaps and ensure continuity of care
✅ Qualifications

High school diploma or GED required; Associate's or Bachelor's degree in healthcare-related field preferred

Prior experience in case management, care coordination, or clinical support strongly preferred

Knowledge of post-acute care needs, medical terminology, and patient navigation

Excellent communication and interpersonal skills

Strong attention to detail and organizational skills

Ability to multitask and work independently in a fast-paced environment
