Registered Nurse Case Manager
OakMed West, our Grand Rapids Michigan division of AmeriCare Medical, provides integrated health service to hospitals, insurance companies and private homes throughout Michigan. AmeriCare is the parent company of AmeriStaff Nursing Services, Sun Medical Equipment and Supplies, RxIV Pharmacy and QCN Home Health Care, delivering a complete and unprecedented home health care service solution.
We are in search of a full-time Registered Nurse Case Manager in the Grand Rapids area. Candidates should have a strong understanding of the patient discharge to homecare process. Knowledge of benefits derived from Workers’ Compensation and the Michigan auto no-fault laws, as it related to injured patients, is a plus. Candidates with Case Management experience at large metropolitan/ regional hospitals or home care companies would be ideal!
Qualified candidates will have exceptional organizational, communication and computer skills, and have the ability to pay attention to the details while providing top notch patient advocacy services. This position provides an excellent salary commensurate with experience.

Essential Duties and Responsibilities
- Assist care management team in transitioning from hospital system to home care by daily review of hospital census, nurture existing customer relationship building and identifying correctly registered workers’ compensation and auto MVA patients in order to promote and secure growth of the joint venture programs within Corewell’s hospital campuses
- Serve as an advocate and educate hospital staff, case managers, patients and families regarding the company's lines of services and products and on the general benefits of post- acute care utilizing available benefits
- Establish and maintain personal relationships with hospital managers and serve as the primary point of contact.
- Review Michigan no fault benefits with patient/families to ensure understanding of their auto policy, answer inquiries to expedite services, assist patients/families with auto claims process and filing, as well as proactively resolve problems
- Secure and deliver documents, physician orders, demographics, pre-admission, UD10 crash reports, insurance assessments, and assigned claims application to ensure that adequate documentation and signatures are received to provide care and to ensure reimbursement of services
- Create a marketing plan by assessing needs, conducting market research on competitors, identifying opportunities for expansion, then develop, present and execute the program strategies to increase market share and meet company goals
- Use quantitative techniques to record and report daily activity, as well as produce monthly and quarterly reports, identifying trends and overall utilization of the program
- Work closely with the DON, ADON, nursing staff and intake department to ensure admission, addressing special circumstances or needs pertinent to the delivery of care to the patient while fostering team work and process improvement throughout the process
- Participate in trauma rounds weekly, perform presentations and coordinate special events to enhance our visibility within the hospital, generate new relationships and provide post- acute solutions
- Exhibit an in-depth understanding of the post-acute processes, healthcare and the home care and private duty industry, especially regarding how the auto insurance and workers’ compensation reimbursement programs relate to the opportunities in the home care and private duty marketplace
- Continually attend training sessions, conferences and trade shows to increase company availability as well as personal knowledge on post-acute care practices, trends and developments
- Other duties as assigned
Qualifications
- Minimum 3-5 years of home health care or related health care, marketing , liaison, or case management experience along with a proven track record of meeting established sales and budget goals
- Bachelor's degree in marketing, business, nursing or health management required; registered nurse is highly desired
- Proven ability to lead, teach and train others in post-acute options as well as generate leads, obtain qualified referrals, manage a territory, and maintain and build relationships with new and existing customers within the hospital system
- Exceptional verbal and written communications, strong business acumen and interpersonal skills
- Proficiency with Microsoft Office and other platforms such as Outlook, Allscripts and Epic
- Local travel required, must have a valid driver's license, reliable transportation, and auto insurance
- This position requires both time in the office and visits to local hospitals.
- Hours are 8:30 - 5:00, Monday - Friday
