POSITION SUMMARY:

The School Based Wellness Center (SBWC) Nurse Practitioner provides direct patient care commensurate to their training and experience, and in collaboration with CCI physicians. The care provided is appropriate for school-based wellness center with a multicultural, multilingual patient population.


As a member of the wellness center’s integrated care team the Nurse Practitioner will fulfill the following KEY FUNCTIONS AND RESPONSIBILITIES towards the provision of patient-centered, coordinated patient care:

  • Examine, diagnose, and treat patients within the parameters of their licensure.
  • Adhere to evidence-based medical guidelines and standards approved by CCI’s Board and Performance Improvement Committee.
  • Carry-out the major responsibility for student’s health care needs including evaluation, management, and follow-up independently or in collaboration with physician consultation or referrals.
  • Adhere to a work schedule that provides extended (after school) hours on a recurring cycle 
  • Provide telehealth visit coverage for other locations, as needed.  
  • Complete electronic medical records documentation in expected time frames.
  • Complete paper encounters during downtime or system maintenance.
  • Participate in peer review and performance improvement activities and audits.
  • Attend and participate in at least three school related activities per year. 
  • Provide referrals internally within school-based wellness centers (mental health counseling, nutrition services, guidance counseling, etc.) and externally based on health-related needs (WIC, Cardiology, etc.).
  • Working on-site is an essential duty for the job and regular, reliable attendance is required. 
  • Other duties or responsibilities as assigned by the Chief Medical Officer or designee.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of state regulations governing advanced practice nursing.
  • Skill in communicating effectively with students and their families, care team members, school staff and administrators.
  • Skill in establishing and maintaining effective working relationships with other employees, patients, and the public.
  • Computer skills required, including knowledge/experience with Electronic Medical Records (EMRs).
  • The ability to speak and write English fluently.  Bilingual (English and Spanish) preferred.
  • Understanding the policies, procedures, and functions associated with Care for Kids, Montgomery Cares, Vaccines for Kids, MCHIP/ Health Choice associated with School Based Wellness center programming.
  • Understanding COMAR regulations.
  • Understanding standardized school forms. 

PRIMARY CARE SPECIALTY AREAS:

Knowledge of the principles and practice of medicine (including preventive care and acute/chronic disease management) in Pediatric Medicine required.  Knowledge of Family Medicine is a plus.

MINIMUM QUALIFICATIONS:

  • Nurse Practitioner certification from an accredited program.
  • Active license issued by the State of Maryland Board of Nursing.
  • Active Maryland CDS license. DEA licensure. Current BLS certification.
  • Ability to relate to culturally diverse patients and communities.
  • Ability to collect adequate patient data, conduct a physical exam, interpret physical findings and manage or refer the student as conditions or protocols dictate.
  • Flexibility to work at different CCI locations.

WORKING CONDITIONS:

  • Proper and professional grooming is always expected.
  • Prolonged periods of standing, bending, or working at a desk on a computer.
  • The ability to lift (up to 15lbs) is required. 
  • Work under stressful conditions within a controlled enclosed environment daily. The work environment involves exposure to potentially dangerous materials and situations that require extensive safety precautions and may include the use of protective equipment.  
  • ADA Category. In accordance with the American with Disabilities Act, it is possible that requirements may be modified to reasonably accommodate disabled individuals. However, no accommodation will be made which may pose serious health or safety risks to the employee or others or which impose undue hardships on the organization. 
  • OSHA Category 1.  Clinical environment. Possibility of exposure to chemicals and infectious body fluids e.g., HPV, HIV, HEP C, HSV.
