The Behavioral Health Liaison supports CBH Clinical Care Managers in the proper procurement of behavioral health services of CBH members. The Behavioral Health Liaison supports utilization management by monitoring services to ensure approved services are being delivered as authorized and are best meeting the clinical needs of CBH members, meeting and talking with members about their behavioral health services and coordinating care.
Essential Functions:
- Coordinate all aspects of referrals as assigned including tracking, documenting referral status, notifying appropriate parties once provider is identified and following up with members/caregivers and/or providers to ensure services have been initiated.
- Coordinate and share information between respective CBH clinical teams and system partners.
- Conduct member outreach to obtain updates on referral status, check-in on member status, provide encouragement, emotional support, and positive feedback to reinforce healthy behaviors.
- Psychoeducation and engagement with CBH members.
- Assist members in identifying and addressing barriers to treatment.
- Conduct follow up with CBH members in the community and by phone.
- Participate in interagency meetings, as needed.
- Maintain an accurate daily log of work completed.
- Maintain documentation in CBH’s electronic system consistent with agency and industry standards
- Come prepared for and actively participate in supervision.
- Attend assigned meetings.
- Cover work when peers are out of the office as assigned and find coverage when absent.
- Contribute to special projects and other duties as assigned.
Team Specific Responsibilities:
- Facilitate referral process for the Children’s Community Based Services team
- Triage referrals for Children’s Services, including but not limited to Family Based Services, Functional Family Therapy, Multisystemic Therapy (all adaptations)
- Manage Family Based Services, Functional Family Therapy, Multisystemic Therapy (all adaptations) waitlist and referral grids
- Monitor network capacity via review and assessment of weekly census reports for FBS, FFT, MST (all adaptations), and Blended Case Management (BCM)
- Provide support for BCM level of care, as needed
- Generate authorizations and notify providers of services approved
- Complete authorization corrections as needed
- Coordinate and communicate with members, families, providers, CBH staff and system partners regarding status of referrals
- Facilitate transfers to a new provider upon request
- Track and close out referrals in a timely manner
- Distribute electronic packets to Clinical Care Managers for review
- Alert Clinical Care Managers when clinical care management intervention is required
- Enter administrative notes in MCO
Position Requirements:
- Education: Bachelor’s degree in Social Work or related field
- License/Certification: PA Child Abuse Clearance required
- Relevant Work Experience: Minimum of 2 years of experience in direct service including assessment, counseling and coordinating services
Skills:
- Familiar with services for adults and children in the managed care system
- Bilingual in Spanish is a plus
- Excellent interpersonal and collaboration skills
- Excellent verbal and written communication skills
- Proficiency with Microsoft Office and able to type 20 words per minute
- Strong organizational skills with the ability to multi-task in a high-volume environment
- Ability to work independently and as part of a team
- Compliant with HIPAA regulations
Work Conditions (including travel, overtime required, physical requirements and occupational exposure):
- Must be able to travel within Philadelphia and surrounding counties.
- Regularly requires walking, standing, stooping, bending, sitting, reaching, pushing, pulling and/or repetitive wrist/hand movements for various lengths of time throughout the day.
- Ability to utilize computers and office equipment to complete daily work responsibilities.
- Regularly works inside in areas that are adequately lighted and ventilated. Some fluctuation in temperature. No protective equipment routinely needed.
- Must be able to adapt to continuous changes/demands of the job.
Philadelphia Residency Requirement:
- The successful candidate must be a current Philadelphia resident or become a resident within six months of hire.
