We're looking for someone to join our team as a Manager, ECM who:

The Manager of Enhanced Care Management (ECM) is responsible for overseeing and leading a team to implement and manage the Enhanced Care Management program at Clinica Sierra Vista, serving Fresno and Kern County. This role involves managing day-to-day activities, ensuring the efficient use of resources, and maintaining high performance standards while meeting the California Department of Health Care Services (DHCS) ECM program requirements. The Manager will work closely with senior leadership, care teams, and external partners to drive team performance, ensure compliance, and contribute to achieving programmatic goals aligned with Medi-Cal Managed Care initiatives.

Essential Functions:


Team Leadership & Supervision:

  • Oversee ECM staff training, including client service specialists, medical assistants, case managers, community health workers, and outreach workers, ensuring compliance with ECM program guidelines.
  • Collaborate with discipline-specific supervisors (such as behavioral health and substance use disorder specialists) for clinical performance management. Set, model, and reinforce clear expectations, foster open communication, and provide individual supervision and problem-solving support.
  • Conduct annual performance evaluations for ECM staff and provide coaching, mentoring, and feedback to support professional development and adherence to DHCS ECM guidelines.
  • Provide guidance and support to ECM program staff to ensure quality patient care, customer service, and coordination of care for internal and external clients.
  • Monitor staff schedules and PTO requests to always ensure adequate coverage across ECM service areas.
  • Ensure timely submission of staff payroll and accounting records to facilitate prompt payroll processing.
  •  Perform introductory and annual employee reviews, competency assessments, and provide constructive feedback to support organizational and professional growth within the ECM framework.

Operational Oversight:

  • Work with the CAO and other department directors to oversee ECM facility infrastructure and operations, ensuring compliance with DHCS, HRSA, and FQHC regulations.
  • Ensure compliance with agency, grant, and regulatory requirements, including OSHA, ADA, CLIA, HIPAA, HRSA, TJC, NCQA, and all ECM-specific standards.
  • Report and address any legal, privacy, safety, or risk management issues related to ECM operations.
  • Monitor, manage, and reconcile all fiscal activities, including ECM staffing, client care expenses, and operational expenditures. Ensure ECM programs operate within grant and Medi-Cal Managed Care budgets and coordinate adjustments with the CAO and grants team as necessary.
  • Track budget, operating, and supply costs for ECM and related programs.
  • Maintain a safe, clean, and functional work environment for ECM staff and clients.

Collaboration & Communication:

  • Serve as a resource to connect ECM clients to comprehensive care services, addressing social determinants of health and promoting whole-person care.
  • Address client concerns, including complaints, problem-solving, and de-escalation, ensuring timely resolution per ECM and agency policies.
  • Represent Clinica Sierra Vista’s ECM program at external meetings with Medi-Cal Managed Care Plans, local social service agencies, and county healthcare departments.
  • Deliver presentations to promote public awareness of ECM services and eligibility criteria.
  • Build and strengthen external relationships to explore funding opportunities, program expansion, and integration with existing FQHC services.

Project Management:

  • Collaborate in planning, directing, implementing, and evaluating ECM program goals and patient care activities while considering the unique needs of Fresno and Kern County.
  • Work with ECM staff to identify emerging patient needs and act as a resource in locating additional program support.
  • Ensure reasonable and predictable in-person attendance to meet ECM program needs across multiple locations.

Problem-Solving & Decision-Making:

  • Analyze and improve key quality indicators to prioritize, organize, and coordinate strategies for ECM program growth, marketing, and patient engagement.
  • Assess processes, workflows, and staffing to ensure efficient and client-centered ECM care delivery.
  • Identify, track, and report organizational and performance improvement activities within the ECM framework.
  • Ensure staff adherence to Clinica Sierra Vista policies and ECM-specific procedures while proactively addressing compliance issues.

Reporting & Analysis:

  • Monitor and report ECM program performance metrics to support continuous improvement efforts and evaluate outcomes based on DHCS requirements.
  • Track and evaluate ECM program success through data-driven decision-making, patient engagement, and quality assurance reviews.

You'll be successful with the following qualifications:

  • Education: Bachelor’s degree in Social Services, Healthcare Administration, Public Health, or a related field required. Master’s degree preferred.
  • Experience: 5 years of experience in a managerial or supervisory role, preferably Clinica Sierra Vista or Medi-Cal program setting, with a strong understanding of ECM requirements and Medi-Cal Managed Care.
  • Strong leadership, organizational, and communication skills.
  •  Ability to motivate and manage a multidisciplinary team to achieve ECM program goals across geographic locations.
  •  Excellent problem-solving and decision-making capabilities.
