Position: Director of Nursing Practice & Excellence 

Overview:

As Monogram Health continues to scale its value-based, home-centered care model, the Director of Nursing Practice & Excellence role is essential to unify and elevate nursing leadership across the enterprise. Reporting directly to the Chief Clinical Officer, the Director of Nursing Practice & Excellence will serve as the leader for all registered nursing functions, overseeing standards of practice, clinical training, care coordination, and regulatory compliance.

The Director of Nursing Practice & Excellence will ensure that nursing workflows, quality initiatives, and training programs align with Monogram’s clinical goals and pod-based care structure. This role will bridge clinical operations with frontline execution by championing documentation integrity, care model evolution, and team-based delivery models in close coordination with the VP of Social Work, Chief of Behavioral Health, Clinical Integration and Implementation Operations Team, Leadership and Development Team, Regional Pod Leadership and Pod Leadership Teams.

This executive will lead a distributed team of RN leaders and support the growth of a high-performing nursing workforce through standardization, innovation, and data-driven improvement. The Director of Nursing Practice & Excellence will also play a critical role in improving patient outcomes, reducing clinician burden, and strengthening Monogram’s ability to scale high-quality care across markets.

Position Summary:

The Director of Nursing Practice & Excellence is responsible for strategic nursing leadership and execution across Monogram Health’s clinical programs, with a focus on care coordination, quality outcomes, clinical training, and regulatory compliance. Reporting directly to the Chief Clinical Officer, the Director of Nursing Practice & Excellence drives operational excellence and clinical integration for nursing functions within our home-based care model, ensuring scalable, high-quality, value-based care delivery.

Key Responsibilities:

  • Develop and lead national nursing strategy aligned with Monogram’s clinical and business goals.
  • Oversee nursing practice standards, clinical competencies, and professional development pathways.
  • Partner with Quality, Compliance, and the VP of Social Work to ensure regulatory adherence and audit readiness.
  • Design and implement workflows that enhance nursing documentation, reduce clinician burden, and improve clinical outcomes along with the clinical integration and implementation teams
  • Serve as a strategic advisor on interdisciplinary team-based care, patient engagement, and chronic disease management.
  • Collaborate with Medical and Operational leadership to optimize care coordination and transitions.
  • Champion workforce satisfaction and retention initiatives for the nursing population.
  • Contribute to enterprise initiatives, including clinical program development, EMR optimization, and new market launches.
  • Evaluate clinical performance metrics and drive continuous quality improvement through data-informed strategies

Qualifications:

  • Active RN license; MSN or DNP preferred but not required.
  • 10+ years of progressive leadership in nursing, preferably in value-based care or home-based models.
  • Proven ability to lead distributed teams across multiple states.
  • Thorough understanding of regulatory frameworks, chronic disease models, and nursing workflows.
  • Evidence of executive presence, change management, and cross-functional collaboration skills.
