About the facility 
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac. 

The Transition Care Program (TCP) provides short-term care and services for older people after they leave hospital. By offering maintenance (low level) therapy and support, the TCP allows older people to continue their recovery out of the hospital setting. Longer-term care arrangements are determined and arranged during the time in TCP 

About the role

The Occupational Therapist provides high quality occupational therapy care to clients and families of Transition Care home-based program and provide clinical leadership within Transition Care and the broader At Home programs.   

Key accountabilities of the role include:

  • Providing high quality occupational therapy care to Transition Care Program home based clients through autonomous use of expert clinical skills
  • Providing high level consultation on professional issues within the organisation to ensure the promotion of occupational therapy professional values and high standards of Occupational Therapy service provision
  • Provide leadership and supervision to Grade 2 Occupational Therapists
