Here is what you will be doing:
The Provincial Palliative Care Program (PPCP) at Ontario Health collaborates with delivery partners to achieve the Ministry of Health’s palliative care priorities, as outlined in the Ontario Provincial Framework for Palliative Care. This role is crucial in implementing the framework, developed under the Compassionate Care Act, 2020, ensuring equitable access to high-quality palliative care across all settings.
The Lead will advance the provincial palliative care education strategy and health service delivery framework by developing and implementing the Provincial Education Plan, including development of clinical guidance and educational materials. This position integrates Models of Care into the provincial delivery framework, supports digital and virtual initiatives, and ensures that education, clinical guidance, and system-level strategies are evidence-informed, person-centered, and aligned with provincial priorities.
This foundational work guides the responsibilities outlined below, and drives continuous improvement in palliative care delivery.
Key Responsibilities:
Developing a comprehensive provincial education plan for palliative care, and integrating several models of care into a unified health service delivery framework
Lead the advancement of the Provincial Palliative Care Education Strategy, including the development of clinical guidance and educational materials
Foster collaboration across Ontario Health teams and external stakeholders
Support digital and virtual care initiatives
Translate complex clinical and policy concepts into accessible educational materials
Monitor and evaluate system-level indicators to inform continuous improvement
Chair and facilitate external advisory committees and task groups
Here is what you will need to be successful:
Education and Experience
Completion of a bachelor’s or master’s degree in a health-related field, or an equivalent combination of education, training and experience
10+ years of overall working experience, with 5+ years of recent Palliative Care experience
Experience leading large-scale healthcare projects is required
Demonstrated understanding of Ontario’s health care system and palliative care legislation, including the Compassionate Care Act, 2020, the Fixing Long-Term Care Act, 2021, and the Ontario Provincial Framework for Palliative Care
Healthcare clinical experience is an asset
Education/Facilitation/Program Design experience are assets
Knowledge and Skills
A demonstrated passion for advancing high quality health care within the context of a system-wide strategy
Strong understanding of the Ontario health care system, and palliative care in Ontario. Palliative care expertise would be a significant asset
Demonstrated project leadership and management skills within a fast-paced environment
Demonstrated ability to conduct literature searches, and compile them into summaries and briefs
Proven ability to develop appropriate relationships with key individuals and organizations to establish trust, credibility, and respect
Strong understanding of the social determinants of health and health equity
Strategic planning, including the ability to develop short-term and longer-term plans that reflect current trends while identifying priorities for relevancy and leadership in the future; ability to operationalize strategies to execute key priorities
Ability to communicate effectively with senior management, and external healthcare organizations, requiring exceptional writing and presentation skills and ability to express complex concepts effectively
Ability to problem-solve and apply critical thinking to involve leadership in discussion that might need to be escalated and further managed to mitigate risk to relationships to ensure the reputations of Ontario Health are upheld
