Kline Galland Home is actively seeking a Social Worker (Discharge Planner) for a well-established team on our Transitional Care Unit (TCU) where patients receive short-term, post hospitalization rehabilitation. The social worker will play an active role as a member of an interdisciplinary team consisting of nurses, doctors, Physical therapists, Occupational therapists, Speech language therapists, and other medical professionals.
The primary role of the social worker is to create a safe discharge plan in coordination with the patient, family, and interdisciplinary team.
- Completion of all required assessments, insurance reviews, and medical chart documentation
- Frequent interdisciplinary communication and meeting coordination
- Caseload management from admission to discharge
- Maintain effective, timely communication with patients and their families
- Understand and meet all government requirements for documentation
Qualifications
- Degree in Social work or related field of study
- 2 years' experience in Human Services or applicable field
- Prior experience in activities a plus
- Ability to manage moving priorities each day
- Strong verbal and written communication skills
- Computer skills for charting and documentation
- Strong team player with the ability to have FUN!
