Community Health Services Network (CHSN) aims to be the leader in providing exceptional, accessible and the appropriate level of pediatric primary care close to home to ensure the health and well-being of our patients, their families, and our dedicated staff. CHSN, a part of our office of Population Health-, was established in 1998 after seeing a growing demand for expanded primary care services closer to home. The CHSN network of community practices consists of 16 primary care locations including three school-based health centers across Ohio, Indiana and Kentucky.
CHSN is now hiring for a Community Health Worker to promote and advocate for all our patients. By coordinating community resources on behalf of patients and families, we are re-imagining how we deliver high-quality care to more kids in our region so they can have the best outcomes, experience, and value.
Community Health Workers will assist with care coordination, appointment scheduling, documentation, community outreach, and in-person home/school visits. This Comm Health Worker will travel to all 16 primary care locations as needed.
Scheduled hours are Monday-Friday on Day Shift. The scheduled hours are flexible as long as we can meet patient needs. No weekends, no holidays, no over-time, no on-call.
JOB QUALIFICATIONS
High school diploma or equivalent
2+ years of work experience in a related job discipline
Preferred: Current Community Health Worker (CHW) certification in the state of Ohio
