I. Major Responsibilities:
1. Conducts eligibility screenings and comprehensive intake and follow up assessments to assess eligibility, gather clinical information, and inform treatment planning.
2. Conducts client evaluations including bio-psychosocial assessment, diagnostic formulation, and initial treatment plan recommendations.
3. Meets with clients to provide individual counseling and assist clients in attaining treatment goals.
4. Writes treatment plans including client’s strengths with behaviorally based goals and objectives. Documents service delivery in accordance with department standards and progress of goals and objectives in progress notes. Refers clients to resources with and outside CHL.
5. Participates in multi-disciplinary team meetings and case discussions. Recognize and seek consultation as needed. Develop a training plan to enhance professional growth.
6. Participates in utilization management, employs Commission on the Accreditation of Rehabilitation Facilities (CARF) standards, and assists as needed in re-accreditation processes.
7. Builds working relationships with agencies and organizations in support of clients’ recovery to facilitate referrals and care coordination.
8. Works closely with MMAT clinical team (Recovery Coach, Nurse, and Psychiatrist) to coordinate client services.

4. Complies with all health and safety regulations and requirements.

6. Maintains, regular, reliable, and predictable attendance.
7. Performs other similar and related duties as required or directed.

II. Position Qualifications:
License/Certification/Education:
Required:
1. Master’s degree in a clinical course of study in behavioral health (social work, psychology, marriage and family therapy, counseling, or related field) that meet the academic requirements for independent licensure in psychology, social work, marriage and family therapy, or mental health counseling in the Commonwealth of Massachusetts.
2. Independent licensure including: Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Counselor OR Licensed Psychologist in the Commonwealth of Massachusetts
1. Master’s degree in Psychology, Social Work, Mental Health Counseling.
2. Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), or Licensed Psychologist in the Commonwealth of Massachusetts
3. Driving is not a requirement.
Preferred:
1. Bilingual preferred.
Experience/Skills:
Required:
1. Ability to perform diagnostic and risk assessments using current accepted tools and practices to determine appropriate level of care.
2. Ability to develop and implement treatment plans which are behaviorally oriented, measurable, and appropriate for the identified level of care.
3. Ability and interest to learn and implement evidence-based models of treatment for opioid addiction.
4. Clinical expertise specific to adults with substance use conditions and co-occurring severe and persistent mental illness with an ability to engage clients effectively and work with other professionals and organizations.
Preferred:
1. A minimum of one year of experience in behavioral health or addiction is preferred.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
III. Physical Demands and Environmental Conditions:
Work is considered sedentary. Position requires work indoors in patient care and office environments.
