As a Spiritual Care Coordinator, you would be able to provide care and comfort to United Hospice's patients and families at a highly vulnerable moment.

Salary for this full-time position is $50,000-60,000 per year, and is commensurate with applicable experience. The position includes visiting patients primarily in Rockland County, our Joe Raso Hospice Residence and on occasion Orange County. The schedule is Monday through Friday from 8:30 to 4:30 with a shared weekend rotation as needed.

Responsibilities:

  • Provide direct spiritual care to patients and families as requested by patient/family
  • Actively participate in coordination of all aspects of patient’s care, in accordance with current professional standards and practice
  • Contributes to patient and family counseling and education
  • Participates in ongoing interdisciplinary comprehensive assessment
  • Documents spiritual care assessments, interventions and outcomes in an appropriate and timely manner following UH documentation standards
  • Contact community clergy on the patient/family's behalf      
  • Assist in the development/planning of hospice related events that contain a spiritual component
  • Participate in QAPI program and hospice sponsored in-service training
  • Participate in/or conduct memorial services or funerals for patients, as requested
  • Assist in bereavement activities as appropriate
  • Maintain Point of Service documentation
  • Accept other responsibilities as assigned
  • Incorporate the organization’s commitment to customer service and continuous quality improvement into daily work habits.

Qualifications:

  • Graduate of an accredited seminary or school of theology or advanced training in pastoral care/counseling
  • Knowledge of emotional effects of death, grief and loss
  • Ability to be open, flexible and ecumenical in ministry
  • Commitment to the hospice concept and its implementation
  • Must be proficient in Microsoft Office Suite
  • Two (2) units of CPE preferred
  • Ability to relate to a diverse population and workforce in a personable and effective manner
  • Must have access to a vehicle to visit patients out in the community

Monday through Friday from 8:30 to 4:30 with a shared weekend rotation as needed.
